Driving on the interstate with cruise control. All of a sudden the car braked (no one in front within eye shot) and cruise control stopped. I had to tap the brake manually then accelerator to get the car back under my control. Ever since then I get ACC, LKAS and other warnings when the car starts up. It seems like the front camera catastrophically failed while driving and almost caused the car to come to a complete stop on the interstate. It was very scary.
Started up the van and the following appears on the dash: ACC problem Collision Mitigation Problem Lake Keep Assist Problem Auto High Beam Problem Safety features unavailable when driving vehicle. In my research this issue has occurred with other owners of the vehicle as well.
I am the original owner of this vehicle and have performed all regular maintenance on schedule. It is our primary family vehicle and now over 120,000 miles. Since purchasing it new in 2018, the fuse for the horn has blown out several times each year. Most recently, the horn has stopped working, but the fuse remains intact. I am at a loss as to how to repair without taking apart the steering wheel which could damage the airbag. This is a serious safety issue as the horn is necessary to avoid accidents. I have spoken to the service department at my dealer and they are unaware of any recalls affecting the horn.
The contact owns a 2019 Honda Odyssey. The contact stated that while driving at an undisclosed speed, the vehicle suddenly shifted into neutral as indicated by the gear shift indicator light. The contact was able to coast to the shoulder of the roadway, where the failure persisted, and the vehicle was unable to shift into drive. The Adaptive Cruise Control, Lane Keep Assist, Collision Mitigation System, Road Departure System, and Transmission Fault Detected messages were displayed. The vehicle was towed to an independent mechanic, who diagnosed a failure with the Transmission Control Unit. The vehicle was not repaired. After investigating the failure, the contact related the failure to NHTSA Campaign Number: 19V299000 (Power Train, Electrical System), but the VIN was not included. The manufacturer was not notified of the failure. The failure mileage was 133,000.
The contact owns a 2019 Honda Odyssey. The contact previously received notification of NHTSA Campaign Number: 23V858000 (Fuel System, Gasoline). The vehicle was taken to the dealer, where the fuel pump module was replaced. The contact stated that while driving at an undisclosed speed, the vehicle stalled. There was no warning light illuminated. The vehicle did not need to be restarted. The vehicle was taken back to the dealer, but was not diagnosed or repaired. The manufacturer was not made aware of the failure. The approximate failure mileage was 103,000.
I was traveling approximately 65 MPH on Interstate 75 Southbound near Cincinnati, Ohio with the flow of traffic. Cruise control was not being used at this time. I felt a sudden loss of acceleration and heard a shrieking sound. I also observed the check engine light illuminate. I immediately signaled and moved toward the right shoulder from the middle left lane. Upon entering the far right lane, the same loss of power and shriek sound happened. I took the exit ramp to Sharon Road and upon releasing the accelerator, the vehicle's engine stalled. I coasted to a stop in the far left turn lane. I personally inspected the vehicle and upon using a scope into the spark plug holes, found that the center rear cylinder shows signs that the piston collided with at least one valve, and the piston does not move with the crankshaft. However, the rear valves do move. The front center piston and valves both move. This suggests a connecting rod issue.
After we took the car in all of a sudden randomly these warning lights will come on and my D for drive keeps flashing. LKAS, ACC, Collision Avoidance, Auto High Beam Problem And brake system warning. Sometimes some of them sometimes all of them. I read it’s a common problem but there hasn’t been a recall on it.
All of the alarm lights are going on at once, immediately when the car is turned on. This includes the Collision Mitigation System Problem, Adaptive Cruise Control System, Lane Keeping Assist System (LKAS), Break System. I am told by my local garage that it is a Odyssey radar system failure, that probably originates in the camera or computer, but that they cannot fix it or get parts for it. It is unclear if these systems work or not.
On 10/4/2025, I was driving with my family on a single lane road around 7:30pm. The van automatically went into neutral "N" gear and there was a message displayed on the dashboard saying it is not safe to drive and asking me to pull over when it is safe to do so. Pressing the gas pedal didn't do anything. Eventually the car came to a stop. The car wouldn't allow me to shift the gear to D or P. A cop who was driving by, saw my situation and nudged the car to the side of the road because it was blocking other traffic. I turned off the car and waited for 5 minutes. Turned the car back on. This time the message displayed as "Emission system problem". I was able to drive the van normally for another 30 miles to get back home. There was no obvious mechanical issues either before or after the car went to neutral. On Monday, I took the car to a honda dealership. While driving there, the car still had "Emission system problem" error. As soon as the person in the dealership turned on the car to get the mileage, the dashboard message went away. They did a diagnostic and said that they got a p0810 code. They reset the code and drove it for 10 miles trying to reproduce the issue. They couldn't reproduce it. I have driven the car for more than a week and I don't have the problem. However, a car automatically going into neutral while still driving on the road, seems to be a very dangerous issue. Please investigate this issue.
It is been only 6 years with 100% city and highway driving and dealer informed me that bushings are already cracking and need to be replaced soon. Car is in mint very good conditions with no other issues.
Something is wrong with the camera and causing all warnings to go off.
A MIL warning was displayed for Emissions System Problem flaging our vehicle for emissions testing and registration. The vehicle skips and does not accelerate well from 20-30mph making turns into a busy street unsafe. The computer reads a DTC of P219A. All of this matches the description in Honda Service Bulletin 21-010 which says it covers 2019 Odyssey of all trim levels. Local Honda dealer (Auto Nation Honda of Chandler in Chandler, AZ) and Honda America both refuse to extend this warenty to our vehicle for repairs. We took it to a third party Honda mechanic for a second opinion and they recommended we follow up on the Service Bulletin 21-010.
Please failure of windshield camera 36160 thr a11 has malfunctioned IDAS
Center camera has gone out. Millions of vans in the same position as me.
The contact owns a 2019 Honda Odyssey. The contact stated that while her son was driving at 70 MPH, he crashed into a deer. The air bags did not deploy. After investigation, the contact linked the failure to NHTSA Campaign Number: 24V064000 (Air Bags). A police report was filed. There was no reported fire. The driver struck his head on the steering wheel and sustained head injuries, and medical attention was provided on the military base. While driving, the vehicle stalled at the entrance of the military base. The vehicle was towed to an auto body repair facility. The vehicle was being recalibrated. The air bag light was illuminated. The mechanic determined there was an issue with the seat belt pretensioner. The mechanic determined that the air bag sensor was not hit during the crash. The contact called the local dealer, who confirmed there were no recalls. The vehicle was not diagnosed or repaired. The manufacturer was not notified. The failure mileage was approximately 77,000. The VIN was not available.
Read Driver side power sliding door, got stuck halfway open. I've had this happen on the passenger side as well, but after turning the car off, putting power doors to manual, and rocking it back and forth, I got it fixed no problem and ended up having to replace a bump sensor on the door and it resolved. I thought that's what was going on when it happened to my driver side door as well. However after I turned the car off, and turned the doors to manual it still wouldn't close. I turned them back to automatic, turned the car back on, and the door proceeded to start going back, it rocked violently then broke the rear window and before stopping again. I read this track roller has been a problem for MANY users of this van and in some cases the entire door has fallen off because of it.
Vehicle has code U3000-49 in the MCU module. All vehicle safety systems have shut down and are no longer working until vehicle is repaired (lane assist, collision mitigation, adaptive cruise control, road departure, etc.) Multiplex Camera Unit is on national back order with no ETA available. Vehicle has 5 warning lights and multiple messages that keeping popping up on the cluster.
We began getting several warnings related to Adaptive Cruise Control, Collision Detection, Lane Departure not working. This was due to the multipurpose camera internal failure. The camera had to be replaced by a authorized Honda Dealer at a cost of $2,200. While the car was drivable no safety features related to the multipurpose camera were available. There were no indications of issues prior to the camera going bad. Due to the cost of replacement I would think that a large portion of drivers do not get the camera replaced and continue to drive the vehicle without use of the safety features provided.
The forward facing camera has failed. A myriad of systems are dependent upon functionality of camera including collision mitigation braking, lane keep assist, lane departure warning, forward collision warning, cruise control, and automatic lighting and high beams. The failure also prompts a transmission system failure, though it's unclear how/why that system would be related. All issues are prompted by warning lights on the dash and the systems are inoperable. Research of the issue suggests it's a common failure across Odyssey and CR-V models with front-facing cameras and related safety systems.
Multi-Purpose camera unit internal electronic failure. Front-facing camera mounted in the windshield. This caused several warning lights including the brake system warning light to illuminate. This failure caused the automatic brake system and other safety features in the car to turn off. The vehicle has been inspected by a mechanic. There were no other warning signs.
What component or system failed or malfunctioned, and is it available for inspection upon request? passenger side sliding door not functioning while vehicle turn on. How was your safety or the safety of others put at risk? passenger on my vehicle can't exit out the vehicle in case of emergency. Has the problem been reproduced or confirmed by a dealer or independent service center? none. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? none. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? none.
Multipurpose camera failure, all advanced safety systems were affected (Adaptive Cruise, Lane Keep Assist, Blind Spot Warning, Rear Cross Traffic Warning, Auto-dim headlights, etc) Also, caused engine light, brake system and transmission system errors to appear but they were cleared by replacing the camera with no apparent damage to the actual systems. Replaced at a dealership for a total cost for parts and labor $2200+.
My engine lights started flashing. I was stranded with my children. Had to coast into a nearby gas station. I’m told it’s a failed monocular camera causing all of it. When I google that it looks like it has happened to SO MANY people! A recall needs to happen.
My 2019 Honda Odyssey EX-L has had sporadic errors happen: Adaptive Cruise Control Problem, Lane Keeping Assist Problem, Collision Mitigation System Problem, Road Departure Mitigation System Problem, Blind Spot Info System Problem. These errors have shown up before sporadically and the dealer, DCH Honda of Oxnard was notified April 27, 2023. They could not replicate and did not respond in writing that something was wrong. The errors went away on their own. 3 weeks ago, the errors showed up again, this time also including a transmission system problem warning. The car was taken to Honda in Valencia and they ran a diagnostic and recommended the transmission be replaced or they would give us $2500 for the car. We opted to have a well reputed private transmission mechanic replace the transmission. After doing so the errors are still there. The transmission system problem error even showed up sporadically the very same day we picked the car up. There is no rhyme or reason to these errors other than perhaps a faulty windshield camera, which takes 5 minutes to replace but Honda wants to charge $2k. We also experienced the delayed shifting due to the poor 9 speed transmission and vcm design. It was quite scary to have a car full of children making a left turn on a busy road and not have the car kick into gear but rather slip. Installing a vcm tuner to bypass the computer shifting helped as well as not driving the car in Econ. Honda knows they made a bad car and while Honda Pilots and other models have had recalls related to the transmission, I am atill waiting for NHTSA to take action and include my lemon Odyssey as well. Replacing the transmission cost me over $7k and it may not have been necessary in the end if in fact these errors were related to the camera.
While traveling in my 2019 Honda Odyssey (40,000 mile) the vehicle completely shut off and placed itself into neutral. The warning lights for emissions system problem, brake problem, and parking assist problem lit up on the dashboard. The vehicle went into neutral and would not allow me to drive at all. Then the vehicle shut off all within 30 seconds. The vehicle would then not restart and now i have to get the vehicle towed to the Honda Service Department. After looking through other complaints there is a known recall for manufacturer defected fuel injectors for my year model odyssey but does not appear when i search for recalls. This was an insanely dangerous situation here in Oahu where the traffic is very bad. The vehicle gave no warning or check engine light and we have completed every service for the vehicle. The vehicle just randomly completely shut off and would not restart leaving my family of 5 stranded.

The contact stated that while driving 65 MPH on the highway and driving over a bump in the road, the side curtain air bags and the passenger’s seat air bag deployed. The contact linked the failure to NHTSA Action Number: PE25018 (Air Bags). There were no reported injuries. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The vehicle was taken to a collision center, which determined there was no impact to the vehicle or internal failure. The manufacturer was contacted and confirmed that the air bag might deploy at 26 MPH. The failure mileage was approximately 110,000.
Front windshield camera failed and available for inspection if needed. All safety features tied to this camera are now disabled. Even basic cruise control is disabled. Problem has been check by a dealer with an estimated cost of $2600 to repair a simple camera module. Not inspected by police but was inspected by the manufactures dealer. I can get it inspected by the police if it helps. There are several warning lights that come up after starting the car. Even dinging when backing out of the driveway causing distractions. This is a known issue for this year Honda Odyssey van. There are lots of posts about it and how to try and fix it without the dealer charging you 10% of the cost of a 2025 model. For something designed for families this is not what I expected. This vehicles safety is impacted by the removal of these features.
Front monocular camera failed for no particular reason. Very unsafe.
The camera in the front windshield went out. It has caused NONE of my safety features to work. Honda is charging $2,500 for the part and labor. It's ridiculous how many people are having the SAME issue. My van didn't even have 52,000 miles. Honda needs to fix this issue

The contact was informed that the failure was possibly due to a failed fuel pump module. The contact was informed that the transmission needed to be replaced. The vehicle was not repaired. The manufacturer was made aware of the failure. The contact was informed that the repair was not covered under warranty or recall. The contact was advised to contact the NHTSA Hotline to report the failure. The failure mileage was approximately 150,000.
On our 2019 Honda Odyssey the passenger sliding door is failing to latch closed and stay closed or locked. The door is popped out in the back because the latch mechanism will not engage and electronically lock as part of the normal closing sequence like the other door. From what I can tell, this is the same issue reported in Service Bulletin 18-128, however when I called Honda about it, they said my VIN was not included in that recall. I am concerned because it seems like the same problem and it creates an unsafe situation where the door is not fully latching, even when manually closing the door and trying to lock it. I can hear excessive noise from that side of the vehicle, and the warning message flashed saying there is an issue with the Right Side Sliding door, and the dome lights come on with an audible warning tone. I am unable to put passengers in the back of the van and needed to be very careful when driving it to the dealership for diagnosis yesterday. I'm still waiting to hear from them. I think this issue should be covered under recall work, since it seem like it is the same issue described in the service bulletin 18-128.
The Camera Assembly Monocular (part 36160-THR-A81) is shorted out causing multiple safety system failures from collision warning, lane departure, emergency braking, etc. Confirmed by local mechanic and then Honda dealer (Merrillville, IN). I still have the part, not yet replaced. Quoted $2K to replace. While driving multiple warning lamps went off and cruise control disengaged. No signs of issues prior to this occurrence. Now none of the safety features related to the ADAS system work.
My Honda Odyssey does phantom braking once in a while. There are no vehicle in front of it but it still brakes. One time it brake till full stop from 40 miles per hour.
I am writing to file a formal complaint regarding a premature and dangerous failure of the high-pressure fuel pump in my 2019 Honda Odyssey, VIN [XXX] . This component failure occurred while my wife was travelling from New Jersey to central New York on Saturday, August 9, 2025, leaving our vehicle disabled and creating a significant safety risk and financial burden. I am formally requesting full reimbursement for the repair and towing costs. On that date, while [XXX] was driving our vehicle through Jim Thorpe, PA, the fuel pump failed suddenly and without warning. The "emissions system problem" and "brake system problem" messages appeared on the dashboard, and the engine began "coughing" and "stuttering" before losing all motive power. This is an unacceptable and extreme safety hazard for a vehicle of this age (70,000 miles). [XXX] was forced to have the vehicle towed to our local Honda service center, Burns Honda in Marlton, NJ. Technicians at Burns Honda diagnosed the failure, noting diagnostic trouble code P0087, indicating that the rail fuel pressure was too low. According to their report, they confirmed an internal fault with the high-pressure fuel pump after observing the fuel pressure had plummeted from a normal 3000 PSI to just 70 PSI. The necessary repair was a full replacement of the high-pressure fuel pump with Honda-genuine part kit 06161-RLV-305. While I understand my specific VIN may not currently be under an active recall for this component, my research shows that Honda is acutely aware of systemic issues with fuel pumps across its lineup. Honda has issued massive recalls (e.g., NHTSA Campaign Number 23V-799) for fuel pumps in millions of 2018-2020 models, including the Odyssey, Accord, and CR-V. The symptoms described in these official recalls (a potential for the engine to stall or lose power while driving) are identical to what we experienced. While accelerating from a complete stop at a red light, immediately after refueling, the vehicle suddenly lost all propulsion power and forced itself into Neutral without any driver input. The dashboard lit up with numerous system warnings at once, including: •Emissions System Problem •Brake System Problem •ABS Problem •Brake Hold System Problem •Electric Parking Brake Problem •Vehicle Stability Assist (VSA) Problem The engine won't start, leaving me stranded in the middle of an active intersection. I attempted to restart the vehicle multiple times with no success. Police responded to the scene, confirmed the vehicle could not be driven, and a tow truck transported it to a nearby Honda dealership. The tow truck driver suspected an alternator failure, but the dealer’s inspection the next day could not reproduce the issue. They reported that the alternator and battery “tested fine” and that no trouble codes were stored in the ECU. No repairs were performed, and no cause was identified. Safety Concern: This incident created an immediate traffic hazard. The vehicle’s sudden loss of propulsion in traffic, combined with the inability to restart, could easily have caused a collision. Multiple critical systems were reported as failed simultaneously, indicating a possible electrical, fuel system, or control module failure. I am aware of similar complaints from other 2018–2019 Honda Odyssey owners in which sudden power loss occurred, often accompanied by numerous system fault messages. Many reports link these events to fuel pump failures, loose battery/ground connections, or transmission control malfunctions. This suggests a broader pattern of intermittent safety-related defects. Requested Action: Please investigate this incident and similar cases for potential design or component defects.
My foward facing multipurpose camera quit work causing all my ADAS systems to suddenly stop working. While researching the issue i found alot of people are having this issue per the honda odyssey online forums. And staff at the local honda dealer that repaired system for over 2500.00 stated that he has seen an increase in this issue with the camera system. My car was no longer cover under warranty and the extended warranty i purchased refused to cover said repair. Just thought maybe honda has a issue with the camera and maybe should find a fix and do a recall before this failure causes an accident.
Sometimes the door locks will not lock, an obvious safety issue. I'm certain all the doors are closed and contacts are clean. I'm guessing a flakey relay because it doesn't happen a lot, perhaps 8 times in the past 12 months. I reported it to Honda America but they said the dealer has to verify it and the dealer has been unable to reproduce it.
My daughter was injured when the third row, far right side seat belt malfunctioned, and she became entrapped. My daughter had fallen asleep in the seat and lay a little sideways resting on a sibling buckled in the center seat. When she arrived home the belt was very tight, and would not undo. She tried to get out and the seat belt auto retractor mechanism was working making it harder to move and then impossible. We reclined the seat a little to try and pull her out and this didn’t help. We also tried removing pieces of clothing and pulling her out to no avail. By this time our child was in pain as the belt was cutting into her waist. Neighbors came out of their homes due to the screaming, this was evening time so felt no option but to cut the belt and release her. This event was very frightening and distressing for our family. We do not understand how the auto retractor and buckle mechanisms malfunctioned but worry it may happen again. We have read similar accounts on the Honda forums. The staff were not helpful or concerned about this malfunction despite a mechanic at Honda acknowledging this has happened before. I would like to formally lodge my concerns over this incident.
Main windsheild camera failed.
When my car hit 40000 miles or so, I started getting all sorts of warning lights showing up when I turned my car on. Took my car in to Honda today, and they said upon checking it that the multifunction camera which is located behind the rear view mirror has stopped working. The car has 44,500 miles on it. It has been in no accidents. There seems to be no reason for such a part to just stop working (needing a $2500 repair to fix).
Service engine light has been lit up and it says “emissions system problem”. I have had it tested at the shop and it states code P219A (air-fuel ratio variation of cylinder [bank 1, rear bank) which I am told has a warranty extension from Honda for 10 years or 150k miles. I am told my VIN does not quality. If other 2019 Odyssey’s have the same issue and qualify, why doesn’t mine? The fuel injectors are clogging and failing way too early for 66k miles. Was Driving on my way to work and I heard a sharp sound while going up a hill. I then got warnings that said transmission issue, pull over immediately when safe and the car shifted itself into neutral. I was in the middle of traffic and lost all acceleration controls, while being forced out of the drive gear. I lost all speed and had to move through traffic and get to a side street. The same thing happened to me about a year and a half ago. It was covered by a warranty but I still was experiencing losses of car power or stalling.
My 2019 Honda Odyssey is experiencing significant fuel system issues that match the symptoms described in NHTSA Recall 21V-215 (Honda fuel pump recall), but my VIN is not currently included. I’ve had repeated problems with: – Engine stalling – Long crank/hard starting – Multiple warning lights – A diagnostic trouble code: P0087 – Fuel Rail/System Pressure Too Low These issues present a serious safety risk—the vehicle has stalled during driving, including with children in the car. Honda dealers say my VIN is not eligible for recall, but the symptoms align exactly with recalled vehicles. I’m filing this complaint in hopes that NHTSA will investigate potential undercoverage of this recall campaign. My family’s safety is at stake.
Not sure if this is the right complaint form, but we attempted to have a service center address 2 recalls on our vehicle and they would not perform them and would not provide a reason why the recalls were not addressed. The service center is AutoNation Honda in Westminster CO [XXX] ). We dropped our vehicle off on 7/24 for an oil change and to address the recalls. They said the work was complete on Monday 7/28 but would not provide an invoice of work completed. I've attempted to contact the service center by text and phone calls and they are not responsive. Camera has failed disabling all the safety features associated and also the cruise control.
we were driving on the freeway when the car it stalled and place itself in neutral. we were unable to put it in park or drive. so we started googling while we were also trying to get a tow truck and we just decided to turn off the car and turn it back on and hoping for the best and we were able to put it back in drive and safely drive home definitely there’s a problem with the transmission or something electronical.

The seat belts retracted as intended during impact, and the lift gate and rear driver's side window were shattered as a result of the crash. In addition, the vehicle occupied one passenger in the front passenger side seat and a second child passenger in the rear row driver's side seat. No warning lights were illuminated. Medical attention was sought for the front passenger for back injuries upon hitting the dashboard during the impact. Medical attention was sought for a strained neck for the driver. The child did not sustain any injuries or seek medical attention. A police report was filed. The local dealer was not contacted. The vehicle was left at the contact residence, where it remained for further investigation. The vehicle was not diagnosed or repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 33,740.
High pressure fuel pump is causing decreased acceleration and lots of noise. Been diagnosed by Honda dealership and other repair shops.
Infotainment system failure which means I am unable to use the baCJ up camera or the hands free link until I replace the entire infotainment center
The sensors in the rear view camera and the grille are malfunctioning causing my van to throw error messages for the adaptive cruise control, lane assist, automatic high beam and brake light to stay on. Theres a risk of accidents happening if the van doesnt notify the driver of lane departure at the right time.
