The contact owns a 2008 Toyota Prius. The contact stated that while driving approximately 15 MPH, the brakes failed when pressed causing the vehicle to crash into the rear end of a second vehicle. During the crash, the air bags did not deploy and the rear driver-side seat belt had sprung outward, although no rear seat passenger was present. The second vehicle's rear driver's side passenger was later treated for an undisclosed injury. A police report was taken at the scene and the vehicle was towed away. The cause of the failure was not yet determined. The manufacturer and local dealer were notified of the failure. The failure mileage was 240,000.

WENT OFF THE ROAD INTO A DITCH TO AVOID A DEER. DITCH WAS IM GUESSING ABOUT 4 FOOT DEEP . AIR BAGS DID NOT WORK. HIT MY HEAD AND HAVE A LARGE BRUISE ON MY CHEST .
STRUCK A CAR AT APPROX 50 MPH SPUN THEN CRASHED INTO A TREE DEAD CENTER OF CAR. AIR BAGS DID NOT DEPLOY.

THE VEHICLE WAS TAKEN TO AN INDEPENDENT MECHANIC WHERE IT WAS DIAGNOSED THAT THE SEAT BELT NEEDED TO BE REPLACED. THE VEHICLE WAS REPAIRED. THE VEHICLE WAS NOT TAKEN TO A DEALER. THE MANUFACTURER WAS NOT NOTIFIED OF THE FAILURE. THE APPROXIMATE FAILURE MILEAGE WAS 143,000.
MY CAR HAS BEEN MAKING NOISES FOR A COUPLE YEARS NOW AND I RECENTLY FOUND OUT IT'S BEEN MY BRAKE ACTUATOR. DUE TO THE ACTUATOR NOT KEEPING COMPRESSION THE ABS BRAKE SYSTEM ISN'T WORKING PROPERLY AND MY CAR ROLLS RIGHT OUT OF THE DRIVEWAY INTO THE ROAD DIE TO POOR BRAKING SYSTEM. I'VE CALLED TOYOTA AND SPOKEN WITH THE DEALERSHIP THEY'RE AWARE OF THE SAFETY ISSUE BUT STATE MY CAR IS OUTSIDE OF THE WARRANTY PERIOD AND WANT $3400 FOR REPAIRS THAN MAY INCREASE ONCE WORK HAS BEGUN.

MY DAUGHTER WAS DRIVING WITH A PASSENGER IN THE FRONT, ON A TOWN STREET AND A VEHICLE PULLED OUT IN FRONT OF HER. THE IMPACT WAS ON HER FRONT BUMPER AND HOOD PUSHING THE SIDE QUARTER PANELS BACK. THE OTHER VEHICLE WAS AN OLDER PONTIAC GRAND PRIS, THEIR AIRBAGS DEPLOYED. NO AIRBAGS DEPLOYED ON THE TOYOTA PRIUS.WHY DID THE AIRBAGS NOT DEPLOY?
I WAS IMPACTED DIRECTLY FROM THE SIDE, WHILE IN MOTION ON A CITY STREET HEADING STRAIGHT. I SLIGHTLY SWERVED TO THE RIGHT. THE OTHER DRIVER RAN A STOP SIGN FROM A CROSS STREET. THE IMPACT WAS HARD ENOUGH TO TOTAL THE VEHICLE, AND TO BOUNCE MY HEAD AGAINST THE DRIVER'S SIDE WINDOW, PUSH ME INTO A CURB ON THE OPPOSITE SIDE AND CROSS TRAFFIC. THE DAMAGE WAS AT THE MAJORITY TOWARD THE REAR DOOR, BUT IMPACTED THE CENTER OF THE CAR AND DRIVER DOOR ENOUGH WHERE IT WAS PUSHED AN 1-INCH AND THE DRIVER DOOR COULD NOT BE OPENED. FIREFIGHTERS AT THE SCENE WERE CONCERNED THAT THE CURTAIN AIRBAGS DID NOT DEPLOY. I AM ALSO CONCERNED. I WOULD LIKE TO KNOW IF THIS IS A DEFECT THAT COULD INVOLVE OTHER CARS AND OTHER PEOPLE IN AN ACCIDENT.
PASSENGER SIDE AIRBAG LIGHT SHOWS OFF WHEN PASSENGER IS RIDING IN PASSENGER SEAT. PRECISION TOYOTA TOLD ME THE WEIGHT LIMIT IS 80 POUNDS TO TURN ON THE AIRBAG. MY WIFE IS BETWEEN 90 AND 95 POUNDS AND SOMETIMES THE AIRBAG WILL SAY ON AND OTHER TIMES IT WILL SAY OFF. I HAD A 2015 PRIUS V FOR THE LAST WEEK AND IT HAS THE SAME PROBLEM WITH THE PASSENGER AIRBAG NOT TURNING ON WHEN MY WIFE IS PRESENT. THIS IS A SAFETY ISSUE, BECAUSE IF SOMETHING WAS TO HAPPEN, THEN THE PASSENGER WILL BE DEAD BECAUSE THE AIRBAG DID NOT WORK. WHEN I WENT TO PICK UP THE CAR, ONE OF THE MANAGER TOLD ME THE AIRBAG WILL NOT BE ACTIVE UNTIL THE PASSENGER IS 150 POUNDS OR MORE. TWO PEOPLE IN THE SAME DEALERSHIP GAVE ME DIFFERENT NUMBER FOR WHEN THE AIRBAG WILL ACTUALLY WORK AND SAVE THE PASSENGERS LIFE. I BELIEVE THIS IS A DESIGN PROBLEM THAT HAS BEEN AROUND SINCE 2008 AND IS STILL THE SAME IN THE 2015 PRIUS V MODEL THAT I HAD THIS PAST WEEK. MY DRIVER SIDE SEAT BELT STOPPED RESPONDING AROUND IN 2015. THE LATCH WORKED CORRECTLY HOWEVER THE SENSOR WOULD SHOW MY SEATBELT WAS NOT DONE. I HAD THIS REPAIRED AT MY EXPENSE IN EARLY 2017 DUE TO WORRY IF THERE WAS AN ACCIDENTAL I WOULD BE UNABLE TO PROVE I WAS WEARING A SEAT BELT.

PASSENGER AIR BAG LIGHT IN INSTRUMENT PANEL REMAINS OFF EVEN WHEN AN ADULT IS IN PASSENGER SEAT, THUS NEGATING THE DEPLOYMENT OF AIR BAG WHEN OTHER SENSORS MIGHT ACTIVATE IT. NOR DOES AN UN-BUCKLED SEAT BELT SENSOR ACTIVATE THE WARNING BUZZER. FYI SIMILAR PROBLEM. TOYOTA IS RECALLING 11,153 OF ITS 2012-13 SCION IQ MODELS BECAUSE THE PASSENGER AIR BAG MAY MALFUNCTION. IN ITS REPORT TO THE AGENCY, TOYOTA SAID THE PROBLEM WAS BECAUSE OF A CABLE UNDER THE PASSENGER SEAT THAT IS PART OF THE SYSTEM THAT DETERMINES IF A PERSON IS SITTING IN THE SEAT AND THE AIR BAG SHOULD BE TURNED ON. THE REPORT SAID THE CABLE COULD BE DAMAGED BY MOVING THE SEAT FORE AND AFT. THAT COULD EITHER CAUSE THE AIR BAG NOT TO DEPLOY IN A CRASH OR TO DEPLOY WITHOUT BEING IN A CRASH. *TR
