The contact owns a 2015 Ford F-150. The contact stated that while driving 30 MPH and attempting to activate the right turn signal, the indicator failed to operate as intended. Upon inspecting the vehicle, the contact became aware of water inside the taillight assembly. The vehicle was taken to a dealer, where it was determined that the taillight assembly needed to be replaced due to condensation. The vehicle was not repaired. The manufacturer was not notified of the failure. The approximate failure mileage was 30,000.
While driving at night a random engine light came on as well as the wrench light came on. Truck downshift and gets stuck at 3rd gear
While driving 75mph with cruise control set the transmission downshifted to 3rd gear. I put it in manual to shift the gear back up to 6th gear. This is the 3rd time it has happened in 6 months the first and second time was while driving in Mississippi on I-10 East and Westbound in the same trip. The last time was Sunday April 5th 2026 at approximately 1145 AM CST while traveling on I65 South in Alabama. No lights came on the dashboard when it happened.
I was driving and came to a stop at a light, let off the brake and then my truck came up with a wrench light and wouldn’t read the transmission gear, cab tool pulled these codes u0402, p0706, p0707, p1702, p1921 all pointing at towards the 6R80 transmission for this lead frame/sensor failure. It will work after the truck sits off for a little while but once you start driving for a few miles it goes back into limp mode. I’ve seen you are looking into this as a recall an would love to see others get help with this issue before it’s to late like me now unfortunately
Transmission downshifts on its own while driving from sixth gear to fifth and won't go back into sixth or lower than 3rd when at a complete stop.
Hard Shifting, will enter limp mode in 2nd and 4th gear (usually will down shift to that gear). The check engine light will flash and I have to pull over, stop, shut off my vehicle and then restart it to resolve. I have already spent roughly $4,000 in repairs and still have continued issues. These issues started at approximately 100,000 miles, transmission was rebuilt at 110,000. The current mileage is 139,400.
Transmission will down shift randomly or not even engage when trying to accelerate. It shudders or harshly shifts.
"My 2015 Ford F-150 is experiencing an intermittent, complete loss of the Output Shaft Speed (OSS) signal within the 6R80 transmission lead frame. While driving at speed, the speedometer suddenly drops to zero, the 'wrench' light illuminates, and the vehicle performs an unintended, harsh downshift into a lower gear (Limp Mode/3rd gear). This creates an immediate safety hazard of rear-wheel lockup or a sudden loss of acceleration in traffic. i took my vehicle to a local repair shop where they could not duplicate the fault. i then called my local ford dealer who informed me unless it would duplicate the fault or show a code they could not repair the vehicle due to it being against warranty policy.
Vehicle unexpectedly down shifting while driving causing vechile to jerk. After parking truck, truck presents a no start condition and unlit gear selector lights. While driving truck will go in and out of limp mode. I have scheduled a appointment to bring the truck in for a diagnostic test 03/31/2026.
My electronic shift on the Fly four wheel system keep dropping in gear while I'm operating the vehicle and it's dragging my back tires this is happening without me operating it
While driving on the highway at approximately 70 mph, the vehicle unexpectedly downshifted without any prior warning or indication of an issue. This sudden loss of speed created a hazardous situation, as the vehicle travelling behind me was forced to swerve quickly to avoid a rear-end collision. Immediately following this, the check engine light illuminated. From that point forward, the transmission would not shift below 3rd gear for the remainder of my commute. This significantly impacted the drivability of the vehicle and made continued driving concerning, particularly in traffic conditions. After arriving at my work, I retrieved the diagnostic trouble code, which was identified as P0721 (Output Shaft Speed Sensor). Notably, there were no prior warning signs, drivability issues, or alerts indicating a potentail failure before this incident occurred. Given the sudden and unpredictable nature of this event, especially at highway speeds, I am concerned about the safety implications of this issue. A failure of this kind, without warning, creates a substantial risk not only to the driver but also to surrounding traffic.
Random, sometime months between issues, transmission and vehicle goes into limp mode. Output speed shaft sensor failure codes. This is a problem with the lead frame connector failure in the transmission (6R80 six speed). This is a common problem and should be a recall.
Driving at highway speeds, transmission shifts into 2nd gear. There is no transmission selection on dash. Shut truck off, now won't start. Truck finally started toke to my local shop and had it repaired.
Wipers failed to operate normally while driving. Wiper started moving very slow then stopped moving even with wet windshield. Upon research the problem was a seized passenger side pivot in the wiper linkage that required replacement of the Wiper Linkage assembly. No warning or noticeable problems until this occurred and vehicle was difficult to operate in rainy conditions.
The vehicle’s original engine began producing excessive white smoke during startup. The vehicle was diagnosed by AutoNation Ford, and it was determined that the passenger-side cylinder head was cracked near the exhaust port. This defect allowed coolant to intrude into the passenger-side turbocharger. Due to the extent of the damage, the engine required full replacement. The engine was replaced with a used motor under a third-party warranty on August 21, 2024. The replacement engine is now experiencing the exact same failure symptoms. Specifically, the vehicle produces white smoke at startup consistent with coolant intrusion. Bozard Ford performed a diagnostic evaluation, completed on February 27, 2026. Their findings determined that the replacement engine has the identical defect: a cracked passenger-side cylinder head near the exhaust port allowing coolant intrusion into the turbocharger. This repeated failure raises significant safety concerns. Coolant intrusion into the turbocharger and combustion chamber may lead to engine misfire, loss of power, potential engine damage while driving, and possible sudden engine failure. Additionally, burning coolant may damage emissions components and create hazardous driving conditions due to reduced visibility from exhaust smoke. The fact that two separate engines in the same vehicle have experienced the exact same cylinder head failure suggests a potential design or manufacturing defect specific to the 2.7L EcoBoost engine. I respectfully request that NHTSA investigate this issue to determine whether this is a broader safety-related defect affecting other 2015 Ford F-150 vehicles equipped with the 2.7L EcoBoost engine. Please let me know if additional documentation, repair records, or diagnostic reports are required.
Auto Transmission makes loud noise. Will not shift gear. Engine warning light comes on. After waiting a few minutes, I restart the engine and it may shift gears properly
While driving at highway speed (60mph), the truck suddenly downshifted to 1st gear causing the rear wheels to lock up until the vehicle slowed to roughly 45mph. At the time of the incident, the truck was not loaded, not towing and weather conditions were cool and dry. Since that incident, the truck hunts for gears, shifts erratically and harshly and will occasionally get stuck in a random gear.
The contact owns a 2015 Ford F-150. The contact stated that the vehicle was chugging while driving. While driving 60 MPH the following day, the vehicle made abnormal whining sounds and lost motive power while depressing the accelerator pedal. The check engine warning light was illuminated. The contact was able to pull over to the side of the road. The contact resumed driving, but the vehicle failed to exceed 30 MPH. The vehicle was taken to Auto Zone, where it was diagnosed with a transmission sensor failure. The dealer was contacted and informed that the VIN was not under recall. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 94,000.
The transmission range sensor has failed on my F150. This made my truck go into limp mode and unable to start. The Ford dealership replaced the lead frame and TRS as it was burnt out. The new TRS then began to automatically downshift while I was driving on the highway. It went from 6 gear to 4th back to 6th and then briefly to 1st making my truck lose power before going back to 6th gear while I was driving at 70 mph. There were no warning lights, a service light had come on just before the incident. The safety issue is a sharp and sudden downshift while driving at high speeds this can cause severe accidents. I understand that the Transmission Range Sensor for my Ford F-150 is under investigation by the NHSTA and in the engineering analysis stage before a recalled is determined. The dealership is looking at the problem as I’ve already spent over $2100 fixing it.
As I was slowing down the truck started to flash the 123456 indicator for the gear. The truck then slipped out of gear and even though I was in drive the truck wouldn’t move. Like I was in neutral. Then with a big bang it started to move again. I stopped and shut the truck off. Started back up and continued to drive to get home. I stopped for a red light and accelerated away from the light and got stuck in the middle of the intersection. I had to shut the truck off in the middle of a state highway then start it again to be able to drive away. It turned out to be a transmission issue from something called the “ Lead Frame Connector.” The dealer says it happens to all of them. It was very sudden and very dangerous.
Hard clunk from first to second shift. The wrench light comes on, telling you to check the manual. Vehicle stays in 3rd gear. Will not shift into 6th gear. Will not downshift into 2nd or 1st gear. stays in 3rd gear at a dead stop. The problem was independently reproduced.

The contact stated that the WRENCH symbol was intermittently illuminated. The contact had taken the vehicle to an independent mechanic, who diagnosed the vehicle and determined that the molded leadframe on the transmission needed to be replaced. The vehicle was not repaired. The manufacturer was not informed of the failure. The failure mileage was approximately 131,000.
The contact owns a 2015 Ford F-150. The contact stated that while driving at an undisclosed speed, the accelerator pedal was depressed; however, the vehicle failed to accelerate as intended. The powertrain symbol was displayed, with a message to check the Owner’s Manual. In addition, the contact stated that the failure had occurred while driving from a stoplight. The transmission remained in third gear. The vehicle was turned off, and the powertrain was reset. The failure was a persistent failure. The vehicle was not diagnosed or repaired by an independent mechanic or the dealer. The manufacturer was made aware of the failure but provided no assistance. The failure mileage was approximately 109,320.
Very harsh downshifts at random speeds. Transmission slips very hard when cold and overall slips or down/up shifts hard randomly and fairly often
Odometer Fraud. The contact's husband purchased a 2015 Ford F-150. The contact stated that while researching the VIN online a day later, the contact discovered that there was a mileage discrepancy. The contact discovered that an auction had previously sold the vehicle to the seller. The vehicle was a private sale. The contact contacted the auction, and it was confirmed that the vehicle was sold to the private seller at 480,000, but at the time of purchase, the mileage was 114,000. The contact made several attempts to contact the private seller, but the private seller ended the communications.
The contact owns a 2015 Ford F-150. The contact stated that while driving 70 MPH on the highway, the gear transmission independently shifted from sixth gear to seventh gear. The vehicle was turned off and restarted, and the vehicle operated as intended. Neither an independent mechanic nor a dealer was contacted. The vehicle was not taken to be diagnosed or repaired. The manufacturer was not notified of the failure. The approximate failure mileage was 211,256.
Was driving my truck to work and all of a sudden it down shifted from gear 6 to gear 4. Then through up the wrench icon on dash. Would not come out of 4th gear. Code was p0720. Output shaft speed sensor. Once code was cleared, it was ok again for about 20 miles then did it again.
Truck is shifting into low gear while at highway speed cause load thump or bang
Rough shift in 4th gear .. transmission bangs going in and out of 4 th gear ..There's been news reports your investigating this issue and that it could possibly cause the transmission to lock up at highway speeds
Transmission slipping and bangs into and out of gears downshifts without doing anything when transmission services there was lots of metal fragments on the magnet2 no change and after having transmission reprogrammed no change
2015 driver door latch opened 2 times and was stuck
My vehicle, a 2015 Ford F150. XLT 3.5 Ecoboost 4x4 SuperCrew has been downshifting intermittently while under load. This has almost caused me to wreck my truck as it has gone from 5th gear at 65 mph to suddenly 1st gear causing the rear drive axle to lock up momentarily on the freeway. This has also happened while accelerating onto the freeway where it will go from 3rd or 4th gear under hard acceleration to suddenly slam back into 1st gear. After this happens it will almost always go back to 3rd gear and will not shift up or down. The only solution I’ve found is to pull over, turn the truck off, remove the key and restart the truck. It will then drive normal for some time until it happens again. I was told by Ford that” oh yeah, it’s your transmission lead frame/ output speed sensor”. If Ford knows about this safety issue, why are they not fixing it on their dime. This is obviously a known issue, but I guess some people will have to lose their lives before Ford will fix this.
truck goes into first gear shifts automatically at any speed
Molded lead frame sudden downshifts to first third or fifth gear would have to pull over to the side of the road/highway and shut the truck off and restart it to get it to go away it went from happening every 50-75 miles to the point where it would happen every 1-5 miles or less when it would down shift to first gear it would lock the rear wheels and almost caused me to get in multiple accidents happened right after I bought the truck 1/27/26 all the way until 3/02/26 when I was forced to get it fixed so I could drive the truck properly
The rear window just completely shattered on it's own. The glass pieces were still mostly connected to each other, but the entire glass panel was cracked into small pieces. The vehicle luckily was parked at the time. But, when my daughter got in the car, she touched the glass with her finger thinking it was ice, and cause a section to completely fall apart, with some glass getting on her (one of the pictures shows this hole). It was the coldest day we had in NJ in a number of years. The front of the truck was facing south and it was cold, but sunny (high temperature that day was 19F, low temp was close to 0F). Around 3:45pm, I needed to drive my daughter somewhere, so I remote started the truck, which I'm not sure if it puts the rear defroster on. It's not known if it cracked before I remote started or not. The rear window has a center sliding window panel, which that center panel was perfectly fine. It had snowed about 5 days before, and snow was still on the ground, but the truck had been thoroughly cleaned and driven since then, with no visible snow or ice anywhere on it. The entire rear window glass was in perfect condition before this, no chips in it or any issues. I gave details to the dealer on their online chat tool early this morning, which I was told someone would contact me back, but I haven't heard anything back yet. I had read online about many other people having the same thing happen with their F150 around the same year as mine, so I wanted to report this, as the window shouldn't just completely shatter, and appears to be a known issue.
The transmission will sporadically shift hard and make a squeal noise on acceleration and deceleration between 20 and 50 MPH. Yes it's available for inspection. It has never stopped completely so the risk factor is unknown. Not reported to dealer or independent mechanic at this time nor has it been inspected by the manufacturer or government agency. There are no warning lights dash messages ever displayed.
Transmission downshifting at highway speed, started to stall, went to limp mode had to manual shift to get to exit

The contact shifted the transmission into neutral(N) and came to a stop at the traffic light before starting to drive. The vehicle was not diagnosed or repaired by an independent mechanic or dealer. The contact related the failure to NHTSA Action Number: EA26001 (Power Train). The manufacturer was not made aware of the failure. The failure mileage was approximately 62,000.
Truck began reviving at high idle after throwing a check engine light, then wrench light as I was driving. I was able to safely get off the road. Shut it down wouldn't restart. I let it sit for 4 hrs, it started then I began driving again as there was no indication of an issue before. At 50mph it happened again. Truck began reviving at recline, check engine and wrench light. Shut it down again, and this process of restarting again hours later as nothing was wrong is still going on, 3 yrs of it. Check engine codes that do come up when it begins again all point to speed sensor and leadframe. Someone said check cam sensors but have no check engine on them at all, only leadframe issues that abruptly occur out the blue. When it happens I can driving at any speed and first thing that happens is the throttle revs to redline and Truck begins shifting abnormal, check engine, wrench, shutdown(thankfully), then hours again now indication of an issue! When i get the truck shut off and off the road, which i had it towed 7 times, show p705,706,707 codes all pertaining to leadframe. Please for the sake ny family and many others recall the 2015-2017 F150s for this $2000 electrical sensor replacement that is putting lives in jeopardy.
I was driving down the road at 45mph and the vehicle suddenly shifted into 1st gear and rapidly slowed down. A malfunction indicator came on, I shut the vehicle off thinking it was a glitch, then the vehicle wouldn't restart. This occured in midday traffic, which could've caused a crash. Never had any previous issues with the vehicle. It cost me $1854 to have this repaired at the Ford dealer, they stated the lead frame connector had failed.
Was driving at 50mph on hiway, all the sudden truck has loss of power and engine shut off, coasted down to stop, put truck in par, stared the car and drove another 5 miles and same thing happened again. No warning lite, no error messages was present when shut off happened nor during OBS scan at later time was. When second shut off happened there was a shift down happened. Not sure if the transmission speed sensor has something to do with engine shut off.
My truck started going into neutral while going in reverse, would rev up going forward and go into neutral or limp mode.
Transmission unexpectedly downshifts to first gear then goes to neutral automatically. Requires driver to pull over and shut engine off for 5 minutes then restart and continue normal operation. In addition to this issue the 3-4 shift lags then slams into gear

The audible alarm to alert that a pedestrian or vehicle was approaching or walking by failed to function as intended. No warning lights were illuminated. The contact stated that the failure obstructed the driver's visibility, which prevented the driver from safely operating the vehicle. The local dealer was not contacted. The manufacturer was not made aware of the issue. The failure mileage was approximately 32,000.
My truck abruptly downshifts to a lower gear, first or second, without any warning on the freeway. Seems like wheels lock up momentarily. There is nothing that notifies me before this happens. Very random occurrence but also very dangerous.
While driving at normal speeds, the transmission hesitates and pops when shifting into 4th gear. The vehicle slows down unexpectedly before shifting and will suddenly downshift without warning. This causes abrupt deceleration and creates a safety concern, especially at highway speeds. The issue occurs intermittently and without driver input. Even when I’m in normal gears it will downshift and cause me to slow and lose control even when I am not breaking. It occurs at just 35 mph- highway speeds.
Hard down shift while driving locking up rear wheels almost cause an accident has happen 3 times now no trouble code can’t figure out what causing engine to down shift extremely hard have not driven truck long distances because of incidents lucky all three times it has happend no one was behind me the most recent time on 1/3/26. I ended up in the ditch with no damages to truck
I was driving down the high way going about 40 to 50 mph and it suddenly shifted down into 1st gear making my tires lock up and then the engine light came on with the out put speed sensor code
When driving the truck, the RPMs will randomly jump up and down without accelerating or shifting gears. At times it will also randomly shift when it doesn't need to shift. It downshifts more than it upshifts. This happens at a steady cruise down a highway and sometimes it is like it takes a minute or two to figure out which gear it actually needs to be in. It seems like the transmission may not be communicating well or something. I am worried that I will pull away from a red light one day and it will decide it doesn't want to shift at all and end up in what could be a terrible intersection situation.
I can have the vehicle available for inspection. I have to hold the truck door shut while driving it when the issue happens. There has been no confirmation of the issue by a dealer or independent service center. There has been no inspection of the problem by the insurance company, manufacture, police or others. This year during cold weather the issue started. I bought the vehicle used. -------------------------------------------------------------------------------------------------------------------------------------------- I have a 2015 f150 and when it gets cold i can open my driver's side door, but when i go to close the door, it won't latch. The inside door handle is really loose, like it's not attached to anything. Once the vehicle heats up, the door handle gets stiffer, like it is connected and I'm able to close the truck door. This only happens when it's extremely cold outside. I was told by a ford dealer that this is not a recall door actuator problem. They think it may be the door cable that connects to the inside door handle. They stated it may or may not be a warranty replacement and not a recall. They say it would be around $149 to diagnose the door problem, and if it's not a warranty replacement, I will have to pay the diagnosis cost and repair cost. They say the only way to know if the door repair is under warranty is for them to go inside the door and diagnose. So this is my concern. I have an extended warranty on the truck and it runs out at 147,000 miles. I'm at 144,000 miles. If I take the vehicle to them and they take the door apart and then say it's not a warranty or recall issue, this could cost a lot of money. With all the problems that the F150 has with the door lock problems when the temperature gets cold, it seems to me that Ford should know right away that my truck door problem is a common problem and it should be covered immediately under a recall or warranty. I bought the truck used. Let me know your thoughts on this.
