NHTSA Owner Complaint Log
This page lists owner-reported complaints filed with NHTSA for the 2021 Ford F-150. Complaints are unverified consumer reports submitted to NHTSA and do not by themselves prove a defect or defect rate.
Data synced from NHTSA on May 4, 2026
The transmission will downshifting and the transmission starts jerking then the engine starts missing for about 30 seconds to a minute but the ford dealership said it's not showing any codes so it's not happening now I can't get it fixed they didn't think having a vehicle that starts missing for no reason isn't a problem but the torque converter won't throw codes and it's attached directly to the crankshaft so it's messing with the engine to get it to start missing
The windshield wipers on my 2021 Ford F-150 are inoperable. The wipers suddenly stopped working with no warning. The vehicle had already received Recall 23S10, which was supposed to replace the defective wiper motor arm. However, the wipers have failed again, suggesting the recall remedy was inadequate or that defective components are still being used. Ford and the servicing dealer refuse to reopen the recall or provide coverage, even though this is clearly the same safety issue. Without functioning wipers, the vehicle is unsafe to drive in rain or snow. I request that NHTSA investigate whether Ford’s recall 23S10 remedy is effective and whether additional corrective action or a new recall should be issued to address continued wiper motor failures on these vehicles.
Started after McGee ford did a service on my truck Apparently they told ford they repaired electrical recalls which I wasn’t having issues with nor did I know They did anything! They did however erase the recalls so that was 8-2024 shortly after my truck experienced major electrical issues, another dealership told me it was gonna be 6k and 4 weeks to fix I was in awe I left with my truck that had no blinkers no air bags and so on 15 safety codes! I battled this for a year and then my parking break locked up while driving 70 on the highway! That’s the recall megee claimed they fixed I’m not rich single dad 100% of rhe time and I sure my kids everywhere daily, I do research call ford find intact i had a recalll so im thinking it will get fixed! 6 weeks later they not fixed i was told 2 days Now heritage and ford won’t fix it they want megee ford the certified service and dealer to pay for it. It’s winter im a contractor in Vermont they let me borrow a Sentra with summer tires on it. Couldn’t make it off my road. I’m still driving my kids around! I almost reck once a day as the whole system is messed up now! I feel like I’m being bullied and picked on because of my nationality
In July 2025, i received Ford's Recall Notice stating that is it possible for fuel pressure and flow from the fuel tank to be reduced due to a fuel pump failure- the same condition that caused my breakdown on 11/5/2025 towing the vehicle to the dealer for repair. Because this repair occurred prior to the official recall repair availability, or remedy as is it referred to. The repair cost was deferred to us because there is no official remedy to the recall, and reimbursements are not being entertained until June of 2026.
Dealer has now had my car for almost 60 days with no resolution Manufacturer Recall Number23S65 NHTSA Recall Number23V896 My asset is basically frozen at this point with zero ETA and any care from FORD. I am willing to trade it in at this point but can’t even do that.
The contact owns a 2021 Ford F-150. The contact stated that while driving at an undisclosed speed in traffic, the vehicle unexpectedly stalled. There was no warning light illuminated. The vehicle did not need to be restarted. The contact received notification of NHTSA Campaign Number: 25V455000 (Fuel System, Gasoline); however, the part to do the recall repair was not yet available. The vehicle was taken to the dealer but was not diagnosed or repaired. The dealer confirmed that parts were unavailable for the recall repair. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was not made aware of the failure. The approximate failure mileage was 60,000. The VIN was not available.
The quad LED headlamps keep going out on the truck. It is the vertical side of the light that goes out. I have seen many other F150 with the same issue.
I have taken my truck to two seperate ford dealerships. Both acknowledging something strange on the transmission with the hesitations, the jerkiness and the hard shifting that happens. The first replaced the trans fluid and the filter. The second dealer replaced the valve body. During the 5th visit. They had me drive around their foreman to show him the problems. While he did feel the jerkiness and hesitations I feel, he said that its pretty much not bad enough yet and that the dealership would be held responsible for the cost of a new transmission if they replaced it and ford deemed it not needing to be replaced. Ford corp told me they wouldn't intervene with their attempt to fix it. The ford dealer sent me home and didn't fix anything the 5th time, even after explaining I felt harsh jerking on the way home to the point it felt like it was kicking me and a bunch of white smoke came out of the tailpipe. I even provided them a video of the white smoke when completely stopped after had been driving for 3 hours. The service on this truck is completely up to date and absolutely no reason at 41k miles I should have all these issue with this transmission. Ford has been resisting to replace this transmission but is willing to sell me a $4k extended warranty. Its getting old. This saga needs to end. Ford needs to just recall these transmissions already. This fight has been ongoing long enough! And ford corporate wont even help me. During the 3rd and 4th visit they replaced the valve body sent me home and the next day I had to bring it back after coming out of my work and transmission fluid being all over the ground right after they "fixed" it. They didnt give me a tow truck but rather made me drive almost 20 miles to their dealership. They claim the transmission pan warped but they just didnt screw the pan on correctly and ensure the pan seal was in good shape.
I started smelling gas in the cabin of the truck when I came to a stop. As I was driving on the freeway I noticed my gas gauge going down rapidly. Within minutes I ran out of gas. Got the truck towed to Ford dealer. After diagnosing the issue I was made aware by Ford service department that one of the high pressure fuel injector cracked and was spraying gas out which caused me to run out of gas. With gas spraying in the engine I was at severe risk of a fire. No warning lights or messages appeared when this happened. After investigating I noticed Ford has a recall for this issue for other vehicles but not the F-150.
Daytime running lights quit working. This happens on numerous f150s with reflector headlights. Transmission issues hard shifting and downshifting that could cause wreck due to sudden slowing of vehicle. Known issue with f150 2021-2023
The contact owns a 2021 Ford F-150. The contact stated that while driving approximately 45 MPH, the contact was cut off by another vehicle that had pulled out in front of the vehicle. The contact depressed the brake pedal with force to keep from crashing into the vehicle. The contact stated that the seat belt pretensioner held firmly in place; however, the passenger's seat belt failed to restrain the passenger in the seat. The contact stated that the passenger almost crashed into the front passenger’s side dashboard. The vehicle was taken to a local dealer, where it was diagnosed, and determined that the passenger’s side seat belt retractor and pretensioner needed to be replaced. The vehicle was not repaired. The contact researched online and related the failure to NHTSA Campaign Number: 21V653000 (Seat Belts); however, the VIN was not included in the recall. The manufacturer was not informed of the failure. The failure mileage was approximately 46,000.
The contact owns a 2021 Ford F-150. The contact stated that upon taking the vehicle into the dealer for regular service, the contact was informed that the rear axle hub was fractured. No warning lights were illuminated. The vehicle was not repaired. After investigating the failure, the contact related the failure to NHTSA Campaign Number: 23V896000 (Power Train); the VIN was included, but the vehicle had not yet been repaired. The manufacturer was notified of the failure, and a case was opened. The failure mileage was 32,000.
Ford has issued a TSB 22-2372 regarding the rear camera image going blank when placing the vehicle in reverse. This issue clears by restarting the vehicle. The cause is a software error in the Image Processing Module and SYNC 4 system. The danger is backing up without being able to see anything besides what is in your mirrors without the software update to fix the issue. Numerous children have already died because of vehicles without the safety of a back-up camera. I took my vehicle to the dealership to have it fixed and was quoted almost $1000 dollars to update the software. I complained and the dealership forwarded my complaint to Ford who gave "assistance" bringing the bill to $361 dollars. My question is why a mandatory recall hasn't been ordered for this obvious safety issue? Luckily, I can easily pay this but what about those who cannot? I asked Ford (case #CXH-xxx) why they are not providing the fix for free since it is a safety issue and they used the excuse of the warranty being expired. Hopefully NHTSA will make Ford fix this safety issue for fellow consumers. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
Rear windshield spontaneously shattered. Reduced vision of rear window. Dealer will not inspect vehicle without paying a fee. Rear window defroster was on and there was cricking sound.
While driving my 2021 Ford F-150 PowerBoost, the vehicle issued a pre-collision warning followed immediately by a restraint system warning. The airbag then deployed without any collision or impact. There was no crash, obstacle, or reason for deployment. The airbag deployment shattered the windshield and caused interior damage. This appears to be a false activation of the pre-collision system and unintended airbag deployment, creating a serious safety hazard. This could easily result in injury or loss of vehicle control. The vehicle needs to be investigated for a potential defect in the airbag system, sensors, or pre-collision assist system.
My 2021 F-150 has burnt-out Quad LED side strips on both headlights. These are daytime running lights, a safety feature. I understand there is no TSB or recall, but many owners have the same issue. I am filing to document a recurring safety defect." Attach a photo of your burnt DRL
Both left and right front vertical LEDs which surround the headlights failed. Ford dealer advised both entire headlight assemblies would have to be replaced at a cost of approximately $3600.00
A certified independent inspection company discovered welding on the main chassis of my Ford F-150. The welding is located on the front left frame area. I purchased the vehicle brand new, and it has never been in any accident or collision. This welding on a critical structural part creates a serious safety risk. Frame integrity may be compromised in normal driving or in the event of an impact. I transport my children daily in this vehicle, and I am afraid to drive it due to the possibility of structural failure. The official Ford dealer inspected the vehicle and kept it for approximately three weeks but provided no explanation or repair. The regional Ford office has delayed their technical report for more than five months with no response. The defect has been confirmed and is permanent. No warnings or lights appeared prior to discovering the issue. This is a manufacturing-related structural defect affecting vehicle safety, stability, and integrity. A proper safety resolution is required.
Running lights on the headlight of the truck continue to go out on the truck.
right running light has went out. yes, it will be available for inspection. lack of lighting causes trouble seeing at night while driving. brought to the dealership was told they would need to change the entire headlight. that woul cost around $2000.00. (both sides are now out). from research the 2021 ford f150 running lights are known for going off. $2000.00 all the time get to be big. the running lights do dim a little right before they go completely out. the running lights are all connected to the head light. there is no way to replace the running lights with out change a $2000.00 head light. not to may people have $4000.00 to replace headlights on a 4 year old truck.
I purchased my vehicle in October 10th of 2025. I drove the vehicle home 2hrs away on the Friday. On Sunday the 12th I started the vehicle with a error of " Park brake fault Service now" I had the car taken to the nearest Ford dealership as it sat for 3 weeks. I was told the recall for the vehicle around this issue has been done twice so Pundmamm Ford the local dealer did not want to do it again. They ended up replacing the driver rear brake caliper bled the brakes and said this fixed it. They did not replace the harness as they said twice repaired was weird that Ford allowed this. I ended up paying for the repairs caused by this recall of over 1000.00 dollars. I have drove it now for two days and already the brake light engaged. Luckily it released and I was able to drive home. I have called Ford on this multiple times as well talked with supervisors and need escalation at this point.
The daytime running lights on the Driver's side of my truck went out, which in the case of my vehicle means replacing the whole headlight assembly, which Ford is stating costs $2,000. I did not realize how much of a problem this is until I started reading online forums where it seems to be a problem with many Ford F-150 owners. Having to replace an entire headlight assembly for the running lights is asinine. This is a safety issue, and Ford should be made aware that they are creating undue hardships for their vehicle owners, as safety issues have to be addressed, and for some unforeseen reason, warranties and extended warranties do not seem to cover this issue, which appears to be very widespread, and most likely a manufacturing defect.
My Rear axle bolt broke in early October. It has now been over 3 months and I have zero update on when my recall will be addressed. I have a rental, but this is beyond the fair and reasonable time frame to repair a recall. The Service center can provide zero update or status of when this will be resolve. I'm making 4 payment on a vehicle that I don't have. What other recourses do we have. Can manufacturers just take months on end to repair when customer are without their personal or work vehicles?
The transmission was slipping in 2nd and 10th gear. This was confirmed by the dealer. A full transmission had to be replaced at 66,000 miles. The vehicle had an excellent maintenance record and well kept. Ford did pay half of the cost but I still ended up spending $6k out of pocket and my vehicle was in the shop for 27 days.
The manufacturer is unwilling or unable to perform recall 23V896 in a timely manner. I have submitted my truck for inspection at an authorized dealer, Gullo Ford of Conroe, TX, on October 6, 2025 per manufacturer requirements. As of this writing, January 14, 2026 I have not been advised of availability of parts to remedy the defect. I am asking that necessary parts be supplied and the recall be performed in a timely manner, as soon as possible at this point.
At a stop light vehicle will turn off and nothing electrical will work. It will have to be restarted, While turning at low speeds the electronic steering completely stops working and almost hit another car. All electrical power failure until modules reset then will start. This all happens randomly so it could happen at any point and cause serious injury. Ford can't seem to find a repair for the intermittent issue.
We have taken our F-150 to the dealership multiple times for a clunking issue under the truck, and have been told that it is related to the recall issued. Although, it has never been repaired. We take it in a couple of weeks ago for the exact same issue that has been going on since October of 2024 and suddenly it is a transmission issue. We are told that they can gladly repair the transmission for $10,000 now that they put us off until the extended warranty expired. I think this is happening a lot, as the exact thing happened to my friend's daughter. Also, it appears upon doing a little research that 2021-2024 F-150s and Explorers have this issue frequently well before they reach the 100,000 mile mark. Many times before the buyer has even paid off the loan for said automobiles. This surely poses a safety risk, as well because you never know when the vehicle is going to fail. Also, my husband took his truck in telling them that it wouldn't go into gear and the shifter had a problem in February of 2024, it took the truck's shifter literally breaking off in the driveway (thank Goodness) and being towed in to the dealership before it was repaired.
The contact owns a 2021 Ford F-150. The contact received notification of NHTSA Campaign Number: 25V455000 (Fuel System Gasoline); however, the part to do the recall repair was not yet available. The local dealer was contacted. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was made aware of the issue. The contact had not experienced a failure.
My truck has had both rear axle bolts broken since October of 2025. This is part of the recall 23S65/23V896 for the max tow axles. My dealer ordered parts in October, yet they're still supposidly on national backorder from Ford. New vehicles are coming off the assembly line with these axles every day, and I can find several OEM parts websites online selling the fixed axles, yet Ford continues to tell me that I need to wait longer. This is a safety issue since both of my rear axle bolts are broken, so if the axle were to slip out of the splines I could lose all power.
There is a known problem with the DRLs burning out, which poses a safety issue. Many people have had this problem causing an expensive repair out of warranty. The entire headlamp assembly has to be replaced.
When making right turn to get on highway accelerate pedal pressed vehicle wont take of after releasing pedal and pressing again few times thn it takes off. Its been twice already i was gonna get rear ended
Truck has 74,000 miles when i notice truck having issues prior to this my front wheels wearing unevenly truck eletric steering light up im being told has to be replaced from what im reading up on this shouldnt be having this type of issue and some models in 2021 did have issue
The contact owns a 2021 Ford F-150. The contact stated that while driving at 35 MPH, the vehicle was shaking erratically. There were no warning lights illuminated. In addition, the gear shifter independently shifted into neutral. The vehicle was taken to the dealer, where it was diagnosed with transmission failure. The contact was informed that the transmission needed to be replaced. The vehicle was not repaired. The manufacturer was made aware of the failure, and a case was filed. The failure mileage was approximately 119,000.
Low pressure fuel pump failure Yes check engine light is on. Manufacture says there isn't a fix available yet and they have no idea when it will be available. My truck is missing and loosing fuel pressure. This is a 2021 truck and its now 2025. This needs to be fixed or another vehicle if they can't fix , but they have offered no solution.
Pulling onto [XXX] , with cars coming at me at 55 MPH or more, the transmission slammed into gear and caused it to stall. Dealership diagnosed CDF failure and recommended replacement of the transmission. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
The contact owns a 2021 Ford F-150. The contact stated that since purchasing the vehicle, there was an abnormal film-like substance visible inside the windshield. The substance obstructed the vision while driving and worsened while driving at night. The vehicle was taken back to the dealer where the vehicle was purchased and detailed the windshield in an effort to remove the film-like substance. Despite the independent mechanics best efforts, the film-like substance remained. The contact called a dealer and was informed that the diagnostic fee to inspect her vehicle would be $300 an hour, and the contact declined the service. The vehicle was not repaired. The manufacturer was notified of the failure and referred the contact back to the dealer for assistance. The failure mileage was approximately 48,000.
I am experience the same exact issue as Recall Reference Number: 23S65.
Ford has released multiple TSBs related to a known design flaw with their 10r80 transmission that causes the transmission to experience hard shifts while moving and often occurs in traffic. The hard shifts can cause the vehicle to stall which creates a very unsafe situation, especially on highways and interstates. The transmission defect has been redesigned in newer vehicles, however, there are thousands if not hundred of thousands of these vehicles on roadways that can break down suddenly causing a major accident. The ford owners forums have been riddle with concerns about this issue related to the CDF drum in the 10r80 transmission.
Vehicle lost all power and stalled while driving. Vehicle also stalled at idle when waiting at an intersection, and another time while waiting at a stop sign. No longer starts. Appears to not be receiving fuel and is returning code P0627 "Fuel Pump A Control Circuit / Open" and will crank but not start. Required towing back to my residence. FordPass sent a "Service Engine Soon Warning" stating "the powertrain control system has detected an electrical fault with the fuel pump and its associated wiring". The symptoms match those in 25V455 but this VIN number is not listed in the affected group.
4 months after taking vehicle in for recall fixes, both recalls on vehicle have not been remediated due to parts not being available. No ETA on availability and get a run around when asking for more info. Hub Shaft Recall stats that driving the vehicle with a broken hub bolt will wear shafts but no loaner was given so I have no choice but to drive vehicle with a broken bolt. Ford customer service told me its ok to drive the vehicle but never stated if they are replacing the entire rear end or just the parts listed on the recall.. What is dirving the vehicle ruins the splines on the shafts. Are they going to replace those as well?
My Ford truck was part of Recall 22S26 / NHTSA Recall 22V250, which required the replacement of faulty windshield wiper arms. On 12/12/2023, I took my vehicle to Karl Flammer Ford for the recall service. The receipt I was given states that both wiper arms were replaced as part of the recall. In 2025, I noticed the driver-side wiper arm was loose and could potentially detach while in use. Upon closer inspection, I found the stamped date code on the wiper arm showed “2021,” the year of my truck. This indicates the part is one of the original faulty components included in the recall and was never replaced, despite the dealer’s claim that it was. A failing wiper arm poses a significant safety hazard because loss of windshield visibility during rain could easily result in a crash. I presented this evidence to Karl Flammer Ford, but they continue to insist the wiper arms were replaced. Ford Motor Company has been unresponsive and unhelpful, repeatedly transferring me between departments and promising call-backs that never come. No progress has been made toward fixing the issue. At this time, the vehicle remains equipped with the recalled part, which is available for inspection upon request. The vehicle has only been reviewed by the dealer; Ford has not conducted any proper inspection or resolution. No police or insurance inspection has taken place. There were no warning lamps or dashboard messages prior to this discovery. The only symptom was the driver-side wiper arm becoming loose, which I observed in 2025. Given Ford’s failure to properly address this recall and the ongoing safety risk, I am requesting that NHTSA investigate this matter and ensure Ford fulfills its obligation to replace the defective parts.
We own a 2021 Ford F-150, that is down since the first of September, 2025, due to recall 23S65. We've been in weekly contact with our local dealer since then. They are doing everything possible to get the parts in to repair our truck, but parts are and have been on backorder, with no ETA from Ford. We have also been in contact with Ford customer service in Dearborn and had an open case with them. They have closed our case because they can't help and told us the local dealer is responsible, then they moved us to recall department now and no longer respond. We feel 4 months is a ridiculous amount of time on Ford's part for these parts to be on and still on backorder. We have no option for help and wondering if you could. It's has been an extremely difficult situation for us to be without our truck for this extended amount of time.
Gear shift selector gets stuck in park, drive & reverse. Unable to move vehicle & blocking traffic for a period of time.
September 2025, we were on a vacation when we noticed that the column gear shifter on our 2021 Ford F150 seemed a bit "loose" when shifting. No issues getting into the appropriate gear…but a weird loose feeling. My husband pulled off the leather boot on the column (not sure if this is what it is called, I believe it keeps dust out) and found the problem. Two missing bolts! Earlier that year, I had found a bolt in the floor of the truck, and, unable to find where it came from (or did it even belong to the truck? I did not know) I put it in a ziploc bag, into the glove box. Turns out that bolt fit. So we had one bolt, and the shifting felt okay. But we still needed one bolt, so I did some internet sleuthing. I found the part (W790628S900) and ordered it, and we installed it when we got home. We theorize that the “ziploc bolt” came loose early in 2025, and the second bolt (never found) came loose on our trip. We know that up until this problem, we had never, ever, removed that dust cover, so we were not responsible for the two bolts coming loose. Components: that "failed" would be the two bolts on the column gear shifter (under the dust cover) Safety: we were very worried that without the two bolts, we would not be able to shift at some point! Confirmed: no, because we were camping on vacation, and did the troubleshooting ourselves Inspected: we will ask our mechanic when we next take the truck in Warning: no, just that we noticed that the column gear shifter was "loose" (again, likely that this was after the 2nd bolt came out).
24E08 for catalytic converter. Ford has a customer service program for other VINs. This is not included. This is a widespread failure based on forms I have read across various year. I have 95k miles and have owned for 4.5 years. They also stated the PCM software needs to be updated related to the catalytic converter. This is about a 3k repair. 24E04 covers this exact issue, but my VIN was on included.
The contact owns a 2021 Ford F-150. The contact received notification of NHTSA Campaign Number: 23V896000 (Power Train); however, the part to do the recall repair was not yet available. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The dealer was made aware of the issue and confirmed that parts were not yet available. The manufacturer was made aware of the issue and confirmed that parts were not yet available. The contact had not experienced a failure.
My truck will not update its internal software that controls everything from windows to ABS braking system. My truck has numerous pending updates, verified by my dealer and FMCO update section, that are not being sent to my truck. The updates are there for a reason, to improve and to prevent issues. I asked my dealer to do them, they said it would cost $1500 to do it since they would not install over the air as they should. I contacted Ford on 9/12/25 and they refused to assist by either paying the dealer and or by using my Ford ESP warranty. My safety is at risk because these updates address transmission issues, braking issues, electical issues etc all that could fail and cause an accident. Add these failure to update with the exisisting fuel pump problem and the truck is a serious problem . I am told by Ford that my updates are "stuck" and that there is nothing that they can assist with and I must take to a dealer and pay for the updates that others get for free. But, should there ever be a "recall" for my trucks failure to update, I can apply for a refund. I have about 12 different updates that will not download to my truck. In the history of the truck, this is not the first time it would not download updates. Shortly after it was built it was taken to Ford and a "forced update for the Bluecruise technology" had to be done. Critical software updates to the trucks main computer that do not initiate is not acceptable and a danger to this truck! Thank You!
The contact owns a 2021 Ford F-150. The contact stated while driving at an undisclosed speed, the brake pedal was depressed, and the contact noticed an abnormal grinding sound. The vehicle was taken to the dealer where it was diagnosed with rear axle failure. The contact was informed that parts were not available. The vehicle was not repaired. In addition, the contact was advised not to drive the vehicle. The contact received notification of NHTSA Campaign Number: 23V896000 (Power Train); however, parts were not yet available. The contact stated that the dealer was contacted several times to schedule an appointment for the recall repair, however the parts were not yet available. The manufacturer was made aware of the failure and a case was filed. The failure mileage was approximately 55,000.
The transmission shifts hard from 1st to 3rd during the first few minutes of operations. When I make the turn out of my resident each morning, it nearly fishtails due to the fat that the transmission stays in 1st gear until around 2500-3000 rpms, then shifts violently to 3rd gear. The tires actually spin. After a few stop and goes, and the transmission has cycled through the gears several times, it shifts normally. I have taken it to the Ford dealer in Royal Palm Beach, FL. They said that there were no recalls, despite there being several complaints from other truck owners about the same issue. Ford knows that the 10 speed transmission in the F150 has shifting issues.
Both Running Daytime Lights have stopped working. This is a common issue with 2021 and up F150's. The issue results in a loss of visibility during daylight hours, creating a safety issue during daytime hours. This also seems to be negligence on the manufacturer's side. My understanding of the issue is a part of the headlight gets too hot under normal operating conditions and results in a melting or possible burning of a section of the headlight assembly. This is also a potential fire hazard from this area which is close to the engine also.
