NHTSA Owner Complaint Log
This page lists owner-reported complaints filed with NHTSA for the 2021 Ford F-150. Complaints are unverified consumer reports submitted to NHTSA and do not by themselves prove a defect or defect rate.
I was towing an empty horse trailer on [XXX]. I had left Clemson, SC and although I had a half tank, because of my long trip I decided to top the tank off before I left. I had no issues on the drive until suddenly, on a hill, in the middle of a construction zone with barriers on both sides and no shoulders, I lost power, in what felt like the transmission had quit. There was no warning at all, no warning lamps, no messages, no suspicious sounds or behavior. The engine was still running, but very rough. I coasted to a stop and shut if off. I was blocking a lane of traffic, and immediately exited the vehicle through the passenger door because cars were not expecting a stalled vehicle and were narrowly missing hitting my trailer, which had hazard lights on. I ended up blocking a lane of traffic for 3 hours after waiting for the first tow truck which did not show up, and then waiting for another. The vehicle was towed to the nearest dealership. They have diagnosed the failure as the low pressure fuel pump related to recall 25S75. There is no Ford directed action to take in order to resolve the issue. The fuel pump will simply be replaced with another at this point. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
The contact owns a 2021 Ford F-150. The contact stated that while reversing, the camera became inoperable and blank, and she crashed into a cement base. Although the pole was in her view. There were no reported injuries, air bag deployment, or police reports filed. No warning lights were illuminated. The vehicle was taken to the local dealer, where it was determined that a software update was needed. The update was not completed. The vehicle was taken to another local dealer, Haley Ford, 10724 US-1, North Chesterfield, VA 23237, who confirmed the remedy was not available for NHTSA Campaign Number: 25V315000 (Back Over Prevention) but later determined it was an over-the-air update. The vehicle had not been fully repaired. The dealer informed her to sign a contingency letter. The manufacturer was contacted, and they referred her to a collision center. The failure mileage was approximately 86,444.
Fords 10r80 transmission is a known failure point. Why are they not recalling it?????? I bought my vehicle used from a ford dealership with 36k miles. Quickly realized it was a lemon but nothing I could do. The biggest issue is the transmission. The transmission does not shift properly, it slips, it has major clunking and hard downshifts sometimes pushing through brake pressure, etc. I fought with two separate dealerships to take a look and after months of them trying to convince me it was fine, they tore it apart and it was damaged. Now the "rebuilt" trans is doing the same thing even leaving me in limp mode just now until I turned the vehicle off and back on. You guys need to do something about this please. Ford is not doing anything and its hurting consumers and going to cause an accident if it hasn't already.
The contact owns a 2021 Ford F-150. The contact received notification of NHTSA Campaign Number: 23V896000 (Power Train); however, the part required for the recall repair was not yet available. The vehicle was taken to the local dealer, where it was determined that the left and right rear axle shaft assemblies had failed and needed to be replaced. The dealer informed the contact that the parts were not yet available for the recall repair and needed to be ordered. The manufacturer was not made aware of the failure. The failure mileage was unknown.
I have a safety recall notice 24S55 / NHTSA recall 24V635 on my F-150. Ford requires 2-3 days to diagnose the problem due to current workload. During this diagnostic period, Ford refuses to provide a rental or loaner, leaving me without transportation. I use the truck for work, so I do not have a way to take the truck in for the recall without coming out of pocket for a replacement vehicle. My understanding is that this is a gray area, as Ford has not issued instructions to stop driving, so are not legally required to provide a rental vehicle during the diagnostic period. The fact remains I would have to take a financial loss to bring my vehicle in for this recall due to the diagnostic time. A simple solution would be for Ford to provide a replacement vehicle during this time. I discussed this with the dealership and the corporate number, to no avail.
The contact owns a 2021 Ford F-150. The contact received notification of NHTSA Campaign Number: 23V896000 (Power Train); however, the part to do the recall repair was not yet available. The contact stated that, upon routine maintenance, it was noticed by the local dealer that the axle shaft bolt had fractured. The vehicle was not repaired. The manufacturer was made aware of the failure. The failure mileage was approximately 33,000.
I had driven the car about 15 residential miles to Home Depot and back. As I was driving into my driveway I smelled smoke and looked forward to see a puff of smoke coming out from the front, driver's side wheel well. I put the car in park and pulled the hook release control under the steering wheel. Suddenly flames were coming out the sides of the hood, about 12" high. I pulled the car quickly away from the house and trees. Put the car in park, as the flames were now engulfing the entire hood. I quickly got away from the car and called 911. The Fire Department is close to my house, yet by the time they arrived the car, including interior cabin, was fully engulfed in flames.
The truck hesitated at a section in the highway where it almost stalled out and I needed to cross over and almost got hit by on coming traffic. Also it has happened multiple times going down the highway the vehicle will start to stop and go it feels like the vehicle is going to stall out. This is very dangerous and I feel like this needs immediate attention
The alternator failed at 60,000 ish miles. Specific mileage can be provided upon request. It appears this is a common problem as alternators and alternator parts are unavailable. I have had to order a custom alternator.
I was driving in a torrential rain storm in Florida. My wipers were on as I have the Platinum edition which has numerous sensors. I heard a strange sound then nothing the wipers stopped and I could not see a thing. I was in traffic so i put my hazard lights on and I indicated to the right and pulled over. I could not drive the vehicle so I had to wait the storm out before I could drive. When I got home I noted that this was a common problem with the 2021 F150 and in Particular the Platinum edition. I called Greenway Ford who told me it was covered under warranty but when I got there they advised me that my vehicle was not part of the recall. They advised that a technician could take a look at $179 to assess the damage. I said its clearly wiper motor as there is no sound at all when you try to turn on the wipers its completely dead. I wondered if any other owners had suffered the same fate. Unfortunately I cant drive the vehicle right now as we have regular storms in Orlando. I have only had the vehicle a few months and I was pretty disappointed in their response as it was really dangerous when this happened. I am not a mechanic but it appears by the VIN my vehicle was not part of the recall. I am reporting this in case there are other owners who suffered the same issue and it may warrant a recall. I thank you in advance for any help you can provide and thank you for keeping us safe.
On my 2021 Ford F-150, the passenger-side LED daytime running light (DRL) has become noticeably dim compared to the driver’s side. This issue is widely reported by other owners of 2021-23 F-150 trucks. The problem is not electrical. Owners who have disassembled the headlight assemblies have found that the fiber-optic style light tube that distributes the LED’s light becomes heat-damaged over time. This burned spot blocks light from passing through the DRL, leaving one side of the vehicle significantly dimmer or out. Safety Concern: DRLs are a safety-critical feature designed to increase vehicle visibility during daytime driving. When one DRL is dim or inoperative, it reduces the truck’s visibility to oncoming drivers and creates an inconsistent lighting pattern that can cause confusion for other road users. This increases the risk of daytime collisions. This is not a cosmetic issue — it directly affects vehicle safety. The failure is common across this model year, but Ford only sells the DRL as part of the entire headlight assembly. The cost of replacement ($800–$1,200 per side) places a heavy burden on owners, especially since the defect appears prematurely and outside warranty coverage. Request: I urge NHTSA to investigate this widespread defect in Ford F-150 headlights. The DRL failure clearly reduces vehicle visibility and poses a safety risk, making it a safety-related defect that warrants further action, including the possibility of a recall or extended coverage. My 2021 Ford F-150 5.0L V8 with the 10-speed automatic transmission has ongoing and severe transmission issues that create a dangerous safety hazard. The vehicle intermittently loses power and feels like it slips completely out of gear while driving, including at highway speeds. This sudden loss of acceleration is unsafe and could easily lead to an accident. The most severe issues occur between 2nd and 4th gear. The truck slams in and out of gear, hesitates, and lurches violently. Shifting often feels like the transmission is “hunting” for the right gear, and hesitation occurs throughout other gear ranges as well. At times, it feels like a new driver learning to operate a manual transmission—jerky, unpredictable, and unsafe. This is not an isolated problem. After researching on multiple owner forums, I found many other 2021 F-150 owners reporting the exact same issues: slipping out of gear, hard shifts between 2nd–4th, hesitation, loss of power, and violent shifting. These widespread complaints strongly suggest a defect in the design or function of this 10-speed transmission. This is not just a drivability issue—it is a critical safety concern. I have lost power while merging and while traveling at highway speeds, leaving me unable to accelerate when needed. In city driving, the hesitation and gear slamming create dangerous situations when crossing intersections or pulling into traffic. Given the number of owners reporting similar problems, I believe this is a systemic defect that requires immediate investigation and action. Ford needs to address this transmission failure before it leads to crashes, injuries, or fatalities.
The contact owns a 2021 Ford F-150. The contact stated that while her husband was stopped, a 2023 Dodge Ram 1500 reversed at approximately 55 MPH, causing the rear to crash into the front of her husband's front of vehicle. No air bags deployed. The contact's husband and grandfather, who was a passenger in the vehicle, were injured. The grandfather sustained neck and facial injuries, primarily around the neck and chin area. The husband suffered injuries to his hands. Medical attention was required for both occupants. A police report was filed, and the vehicle was towed. The insurance company was pending an inspection to determine if the vehicle was destroyed or not. The contact informed the dealer of the incident, and referred the contact to the manufacturer. The vehicle was not diagnosed or repaired. The manufacturer was notified of the failure and referred the contact to the NHTSA Hotline. The failure mileage was approximately 59,000.
Left Side Day Time Driving Light went out on top half on my 2021 F150 Ford Truck. Ford has many complaints of this happening to the Day Time Driving Lights from 2021 to 2024. The light is a complete replacement of the entire head light assembly for $ 2000 at the dealer. Day Time Driving Lights are a Safety Feature of Automobiles and should be part of warranties. This is a major problem with the Ford lighting system and it shouldn't cost $2000 to replace a daytime driving light. Being a Safety Item it should be recalled and Ford should be made to replace these head light assemblies to ensure the safety of all drivers.
Vehicle: 2021 Ford F-150 Component: Exterior Lighting – Daytime Running Lights (DRLs) / Headlights (High Beams) Safety Concern: Vehicle visibility and forward illumination Both daytime running lights (DRLs) on my 2021 Ford F-150 have failed completely. After the DRLs stopped working, the high beams also became inoperative, leaving only the low beams working. The Ford dealer attempted a repair by replacing the control modules, but that did not fix the issue. They advised that the entire headlight assemblies need to be replaced. However, these assemblies are very expensive, so I have not replaced them yet. I am currently limited to using only the low beams. Without DRLs, the truck is less visible during daytime driving, increasing the risk of a collision. Without high beams, nighttime visibility is reduced, especially on rural or dark roads. This failure affects federally required lighting functions, compromising safety in both day and night driving. The problem is not normal wear: these are sealed LED assemblies that should last the life of the vehicle. The fact that both assemblies failed strongly indicates a manufacturing or design defect. Owners face high out-of-pocket costs because the only fix offered is full assembly replacement. Other 2021 F-150 owners have reported similar failures of DRLs and headlights online, showing this is not an isolated incident. I request that NHTSA investigate this issue as a safety defect and require Ford to issue a recall to replace the defective headlight assemblies at no cost to owners.
Catalytic converter has failed and needs replaced

Power steering rack failure, while driving though a puddle less than 6 inches deep. Cost over $1900 to replace. My wife's 2014 volkswagen passat is able to drive though deeper water.. on 8/13 I was unable to move over on a road that had some puddles due to oncoming traffic. After driving though the small puddle my power steering went out. Took to the mechanic and he said its a known issue due to how Ford did not seal their wiring well under the trucks and it hangs lower allowing the water to seep into the cables and cause the powering steering rack to fail.. this is unacceptable its a truck a puddle should not cause a truck to loose power steering.
The DRL has gone out on one of my headlights. The other has started to flicker. This is uncovered by their extended warranty and over $1600 per side to replace. It is becoming common to see many 2021-2023 with LED reflector headlamps with burned out DRL. It is an unserviceable part as well. Only can replace the entire assembly but no warranty will cover it as lighting is exempt as standard wear items.

Fords Sync system fails to connect to internet and won’t update resulting in transmission failing to relearn as you drive
Random times my clockspring makes clicking sounds and the car iant able to be driven. Getting many messages about issues with the vehicle
Vehicle: 2021 Ford F-150 (VIN: [XXX] ). The vehicle has been undrivable due to since Aug 4, 2025 and is currently at a Ford dealer in Amarillo, TX. Ford/dealer state it is undrivable due to a recall condition and that parts are not available / no ETA for remedy. Component: rear axle nuts and axle shafts, axle shafts are stripped, may cause rollaway or power loss while driving ,according to Ford. The vehicle has been inspected by Ford at Tri-State Ford in Amarillo, TX where the vehicle has been held since August 4th, 2025. Ford says the axles are under the recall, the recall claims parts are available, but the Amarillo Ford (Tri-State Ford) has NOT fixed the vehicle and states they have no idea when they will get the parts. Ford also sent a message several months ago saying they have no ideal when the parts will be available. Request: NHTSA review the recall remedy delay leaving vehicles undriveable for extended periods. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

Last Monday, I was driving [XXX] near Jacksonville, Fl. While in an outer lane the truck pulled to the right during rain, not like a hydroplaning. I could not regain control and hit a barrier. Airbags deployed. After, I got out saw the rear passenger tire was somehow off. When PSA arrive they found it ~200 feet down the road. It had snapped off. Complete break at the axle hub. Given the recall issued on 2023-2025 models I thought I should report this. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

Getting multiple messages like service advancetrac, pre collision assistance not available, and steering assist fault. Vehicle makes noice randomly when turning but not every time
On Sunday, [XXX], I was driving on a state highway in my 21 Ford F150 FX4 with my family. 67mph, 73 degrees sunny outside. The front portion of my moonroof/sunroof exploded, shooting glass shards internally all over my truck/passengers and externally into the bed of my truck, and into the boat I was towing. Multiple people had glass shards embedded in their skin. There were no other vehicles nearby, and there was no prior damage to the sunroof/moonroof of the vehicle. The truck gave no warning or signs of malfunction prior to the event. The sunroof was closed, and all windows in the vehicle were closed. On Monday morning ([XXX]), I took my truck to the local Ford service center, and they inspected it, shocked at the results. They filed a claim that Ford denied because the windows are not covered by my extended warranty. They wanted to charge me $ 4,000 for the repair/replacement of the glass and track. I have been on the phone with Ford Protection and Ford Customer Service (Aug 6th, 2025), trying to get something resolved, and Ford continues to deny responsibility. The DRL on the passenger side of my truck have already begun to go out, I have noticed a ton of these 14th generation F150's on the road with dead/dying DRL and mine began to die after 4 years and 61k miles.
I have been trying to get a recall situated with Ford now for almost a year. They say they have no fix. I am making payments on truck and can’t use it like I should be. Even came out of my own pocket and had ford install a new fuel pump however problem came back. Just not fair I purchased a vehicle from A reputable company and they will not help me at all. Not even a loaner.
Daytime running lamp failure and fog light failure. Daytime running lamp failure causes poor visibility and the failure defect defeats its entire purpose for use. Fog light failure creates poor visibility during fog conditions and could result in not being visible to other drivers.
The LEDs in the daytime running lights have failed. They are partially burned out. Based on research, I’ve done online this is apparently a common problem.
Reference NHTSA Safety Recall 23V896. On our truck, the right rear axle hub bolt broke the last week of July 2025 (exact date unknown; bolt head caught behind hub cap). I made a an appointment with our local Ford service on August 4. To date (January 6, 2026 or 5 months), the repair has not been completed because the replacement parts are backordered nationwide. Per the recall notice, driving the truck risks further damage and possible injury or crash. Anecdotally (online forums and other resources) and despite the reported backorder, the recalls are being completed in a timely fashion in parts of the country. Some recalls apparently are being completed even on trucks where the bolts have yet not failed. Obviously parts are available because Ford is selling new trucks which presumably are not subject to the recall. In December, we were 14,000+ "in line" for the parts according to Ford service. Apparently we moved up about 1,000 places in one month. At that rate, we think it is unacceptable to not have full use of our truck until 2027, are frustrated to have to continue to pay insurance and licensing costs, and are very disappointed at the prospect of canceling summer plans again.
The contact owns a 2021 Ford F-150. The contact stated that while driving at various speeds, the vehicle made an abnormal metallic sound. The vehicle was taken to a local independent mechanic, where the brake pads were replaced; however, the failure persisted. The vehicle was taken back to the mechanic, where it was determined that the rear axle hub bolt had fractured. The vehicle was taken to the dealer to be repaired under NHTSA Campaign Number: 23V896000 (Power Train); however, the dealer informed the contact that the recall part was on backorder and would be available in October. The vehicle was not repaired. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was notified of the failure, and a case was filed; however, no assistance was provided. The failure mileage was approximately 82,500.
The contact owns a 2021 Ford F-150. The contact received notifications of NHTSA Campaign Numbers: 23V896000 (POWER TRAIN) and 25V455000 (FUEL SYSTEM, GASOLINE). The vehicle was not repaired. Most recently, the contact called the dealer and was informed that no appointments were available. The manufacturer was not made aware of the failure. The failure mileage was approximately 43,000.
We had our truck in SEVERAL times when under warranty for repair because the truck back axle was jumping and jerking at low speeds (30-40). of course our luck, they can't find anything wrong. then it goes out of warranty and the back right axle goes out and it is undriveable. We repair it at an expense of 1400.00 in 2024 and now the other back axle went out and it cost 1200.00. Talking with other Ford owners and mechanics, this is a known issue and Ford isn't doing anything about it. Is there any help we can get in matter, Ford says there isn't.

About 1 hour into a commute, I attempted to connect to a call while using the Car Play function of the SYNC system. Upon connection, the speakers blasted as loud as they could possibly go, a terrible pulsing static sounds that scared me and the passenger and almost caused us to have an accident. The volume knob was indicating 0 and would not move and I could not lower the volume of the terrible sound. I pulled over and tried to restart the car a few times, but the sound would not stop until I left the car off for about 10 minutes. I've not connected my phone via Bluetooth or Car Play since for fear that the sound may return when I am not expecting it, startle me and cause an accident. Though I am not certain of the cause. After about 10 minutes of the sound, I also noticed that the speakers were getting hot and I could smell the electronics or wiring overheating. I have a video of the sound but the dealer could not replicate it.
The contact owns a 2021 Ford F-150. The contact stated that while the vehicle was parked, there was fluid leaking from the front of the vehicle. The vehicle was taken to the dealer, where it was diagnosed that the gasket on the rear differential had failed and needed to be replaced. The vehicle was not repaired. The vehicle was taken to an independent mechanic, where the vehicle was diagnosed that the driver’s and passenger’s side rear axle shaft assemblies had failed and needed to be replaced. The vehicle was not repaired. The manufacturer was made aware of the failure and referred the contact to the NHTSA Hotline for assistance. The approximate failure mileage was 69,000.
The contact owns a 2021 Ford F-150. The contact stated that three months after purchasing the vehicle, while her husband was driving, the vehicle was driving roughly, and the transmission unexpectedly downshifted. The contact was unaware whether any warning lights illuminated. The vehicle was taken to the dealer, where it was diagnosed, and it was determined that there was a misfire in cylinders #1, #3, #4, and #6. The spark plugs were replaced, and the vehicle was repaired. The contact stated that on a separate occasion, a month after the repair, there was an abnormal sound coming from the steering wheel. The vehicle was taken back to the dealer, where it was diagnosed that the wheel bearing and axle had failed and needed to be replaced. The vehicle remained at the dealer unrepaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 56,000.
Mostly when pulling out of parking lot but entering traffic at slow speeds complete loss of power for 4 to 5 seconds or longer.
Brought the truck in to dealership July 22 for a recall and they said 1 day till ready so we left it. Now it is still sitting at the dealership 62 days later and still waiting on parts 23S65 rear axle half shaft. 23V896.
My truck completely shut down while leaving the shop and I was only going about 6-10mph. The dash alert stated "fuel accessory" or something in orange, sorry, I panicked a little:) I took it to Bradley Ford the next day and they couldn't fix it as they didn't know the issue. This happened a second time, but didn't shut off completely, it just felt like I was running out gas (full tank). I do live in Lake Havasu City, Az and it's hot here so I don't let my truck go below 1/2 tank hoping this will help avoid a major accident. Thank you! When driving across railroad tracks, or at lower speeds primarily when turning sharp or into parking spaces, a power steering assist fault light flashes and steering becomes difficult unless truck is moving. The only remedy that I know is to put the truck in park, turn off, and restart. This corrects the issue until the criteria mentioned above is met and then it happens again, and again...... The vehicle has been taken to a repair shop where they recommended a new rack and pinion. They mentioned this is a very common issue with ford vehicles. This vehicle has 34k miles and has been well maintained. I'm very disappointed that it consistently has this issue and worry about the loss of steering at a higher rate of speed.
Complaint Regarding Wiper Motor Recall – 2021 Ford F-150 VIN [XXX] I am filing this complaint regarding a serious safety issue with my 2021 Ford F-150 (VIN [XXX] ). The wipers sometimes stop mid-sweep or malfunction during operation, creating a major hazard during rain and poor visibility. This appears to be related to Ford Recall 22S12, which addresses defective wiper motors on this vehicle model. I contacted a local Ford dealership and asked if this recall had been completed. They could not confirm it. Instead, they told me that the problem might have been caused by an oil change shop replacing the wiper blades — and that I would be responsible for paying $200 for an inspection and up to $400 for repairs, even though the issue appears to be recall-related. Replacing wiper blades is routine maintenance and should not affect the motor’s function. I can find no technical data or public documentation that supports the dealership’s claim that improper blade installation causes a wiper motor to fail or stop mid-cycle. This sounds like an excuse to avoid honoring the recall. I am unsure whether the recall was ever completed on my truck. If it wasn’t, it needs to be done now. If it was, and the motor is still failing, that indicates a faulty or incomplete recall repair. Either way, Ford should not charge me for diagnosing or fixing a known recall-related defect. I am requesting: •Written confirmation of whether Recall 22S12 was completed •Immediate inspection and proper repair if the motor was not replaced •Coverage under recall or warranty if the part remains defective I’m also reporting this to the Better Business Bureau to document what I believe is an unfair attempt to shift responsibility and costs for a known safety issue. Sincerely, [XXX] [XXX] [XXX] VIN: [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

I own a 2021 Ford F-150 [Lariat PowerBoost] affected by recall 23S65 (rear axle bolt). The bolts have severed. Ford has advised that my truck is unsafe to drive until repaired. When I brought the vehicle in, I was told parts were available. I drove the car home and later discovered from paperwork that the repair was not completed only the parts had been ordered. I was told not to take the Truck on my upcoming vacation pulling my camper. I opened a case with Ford Customer Care and was originally told parts would arrive before my scheduled use, but now I am told they will not be available for weeks. This creates a serious safety and hardship issue. My family is scheduled to leave for a trip this Thursday that requires this vehicle to transport my family and tow our camper. Ford has not provided a rental or loaner vehicle capable of replacing my truck, despite advising it is unsafe to drive. I am requesting that NHTSA investigate Ford’s failure to provide a timely remedy or interim solution for a safety-critical recall that removes my vehicle from safe use.
