The computer is not working which connects to the forward collision warning system, navigation, and all safety when it comes to driving. I am still waiting to hear back from Tesla about the problem and haven't had a resolution.
SUDDEN ACCELERATION, I WAS DRIVING IN MY OWN DRIVEWAY AND A MODEST SPEED T15 MPH I HAVE A CIRCULAR DRIVEWAY AND I WAS ASCENDING UP THE CIRCLE WHEN THE CAR SUDDENLY ACCELERATED WITHOUT MY ACTION. i TOOK MYI FOOT OFF THE ACCELERATOR AND HIT THE BREAK BUT THE CAR DID NOT STOP OR SLOW DOWN. IT CONTINUED UP MY DRIVEWAY ON MY LAWN AND ACCROSS MY LAWN THROUGH A FENCE AND I WOUND UP IN MY NEIGHBORS LOT WITH CONSIDERABLE DAMAGE. I WAS SHAKEN BUT DID NOT SEEK MEDICAL ATTENTION AT THE TIME. MY NEIGHBOR AND MY WIFE ASSISTED. THE CAR WAS TOWED TO TESLA CONTRACTOR BODY SHOP -RECON. WHERE IT IS NOW. I HAVE REQUESTED THAT TESLA EVALUATE AND REPAIR THE CAR BEFORE I DRIVE IT AGAIN AND PICK IT UP FROM THE BODY SHOP. I NOTIFIED THE BODY SHOPP(TONI MCCARTHY) AND TESLA SERVICE(RADCLIFFE SERVICE PERSON WORKS FOR DON HADDON SERVICE MANAGER) .SO FAR NOTHING HAS HAPPENED . TESLA ASKED ME TO SEND A PHOTO OF THE ACCELERATOR BUT SINCE THE CAR IS STILL IN THEIR AUTHORIZED BODY SHOP, I AM UNABLE TO DO IT. i AM AFRAID TO DRIVE THE CAR AND ASKED THEM TO TOW IT FROM THE BODY SHOP TO THE ROCKVILLE TESLA DEALER BUT THEY SEEM RELUCTANT TO DO THIS. MY WIFE HAD A SEPARATE PRIOR EXPERIENCE WITH SUDDEN ACCELERATION .N HOPING FOR NHTSA INTERVENTION
• Make: Tesla • Model: Model Y • Year: 2024 Description of the Problem: After a software update pushed by Tesla, the AP4 computer in my 2024 Tesla Model Y shorted out, causing a complete failure of all systems controlled by this computer. The following safety-critical and driver-assist features have become non-functional: Cruise Control (adaptive or otherwise) Forward Collision Warning Lane Departure Warning Lane Keeping Assistance Blind Spot Warning Parking Collision Warning Full Self-Driving and Auto Park features Cameras required for Autopilot functionality Additionally, because the GPS system has failed, the vehicle’s Navigation system is inoperable. This impacts Tesla’s unique route planning feature that sets up Supercharger stops during long journeys. Without a functioning navigation system, I cannot properly plan my travel or locate charging stations, which severely affects the usability of the vehicle. Safety Concerns: The failure of these systems, especially safety features like forward collision warning, lane keeping assistance, and blind spot monitoring, puts both the driver and others on the road at risk. Losing access to safety-critical functions after an update is unacceptable and poses a serious danger to vehicle occupants and surrounding traffic. Incident Details: • The issue occurred immediately following a software update. • Tesla pushed the update remotely without providing any warning or indication of potential risks. • The failure appears to be related to a hardware short in the AP4 computer, directly caused or exacerbated by the update. Desired Outcome: • A thorough investigation into Tesla’s software update process and its effect on vehicle hardware and safety systems. • A resolution that prevents similar failures in other vehicles and ensures system updates do not compromise vehicle safety or critical functionality.
My navigation and all cameras abruptly stopped working when I was headed home from work on 12/2. When I entered the information for Tesla service it was automatically switched to AP4 computer short. I told the service center this Tesla is not safe to drive and is lacking all safety feature and asked for a loaner, but was repeatedly denied. My service appointment continued to be pushed by from 12/6 to 12/16 to 1/6 now. When I asked again for a loaner I was told no and that the issue was widespread causing the computers to be in back order.
The cameras stopped working on my brand new Model Y Tesla. I filed a service request Dec 4th and it keeps getting pushed back (Currently Dec 26th with no ETA on part availability) because this part has a widespread issue on newer Model Ys. They are saying it is “Fried AP4 computer, remove and replace”. I just purchased it from Tesla in November. It only has around 1500 miles on it. I can’t use any cameras on my car including backup cameras, forward facing cameras, side cameras. No forward or rear warning or brake assist items work. It’s incredibly difficult to see out of the rear window when backing up without the cameras. I fear it’s a matter of time before I run over a little kid when backing up because the cameras don’t work. See many reddit threads about this issue: [XXX] [XXX] [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
No cams working, no reverse cam, no 360 view, no auto wipers, no regular cruise, no FSD, no ASS or dumb summoning, no emergency braking, no park assist and for some reason no navigation
My New 2024 Model Y Tesla is having a computer failure - no navigation / GPS (stuck at last address when issue occurred) and no cameras (including rear camera). With limited rear windshield visibility and no backup camera functionality, we've been put in dangerous situations. They keep pushing out the service dates waiting on new parts / computers. I found this article that details the widespread issue and parts shortage. [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
Tesla shared I have an AP-4 computer short that disables all cameras (rear back up camera, blind spot camera, lane deviation detection, emergency assisted forward braking), navigation, and auto lighting features are disabled. Other non-essential features do not work as well.
All camera and navigation systems stopped working on 11/30. With no cameras working, there are significant blind spots when driving. I cannot see anything when the car is in reverse due to no backup camera and the mirrors are stuck in a dark "tinted mode" further decreasing visibility when driving. The local Tesla service center says they do not have parts in stock to fix the issue. They have repeatedly delayed any scheduled service appointments, without offering any solutions. The car feels very unsafe to drive. There were no prior warnings of the issue, it just stopped working.
Tesla has told me the AP4 computer has failed in my car which has resulted in them having to replace it. I had this issue start right around Thanksgiving. I scheduled the soonest appointment available for 12/16 however Tesla contacted me and moved my appointment because they stated that the part was unavailable at this time and should be delivered by 01/20/25. Not offering me a rental or anything on my brand new '24 Tesla. It has just over 1,000 miles on it and is ~3 months old now. This issue impacts: emergency braking, blind spot detection, lane keep/centering, assisting with parking, automatic high beams and wipers, cruise control/ adaptive cruise control, Vehicles built in GPS to be stuck at one location every time, has made my touchscreen not automatically brighten or dim, my auto dimming mirrors be to dim in most driving conditions, constantly fail to install system improvements and safety updates, and because my vehicle keeps trying to do this, it has caused increased battery drain as well. This has increased the safety risk of my vehicle tremendously... Tesla has not seemed to publicly acknowledge the issue yet either.
Hello, I have a 2024 Tesla Model Y (Fremont Build) that was purchased in early September 2014. On or around November 30th a software update "fried" the computer according to Tesla. The call the computer AP4... Anyway, they have no ability to fix the issue because of lack of parts. They make an appt for you and then keep cancelling it. I've had two so far that they cancelled because of not having parts. There are thousands of users on Reddit complaining about the same thing. My specific complaint, is this is a safety issue for the Model Y in particular because the mirrors and rear window are inadequate for this size of a car. Visibility to the sides and rear of the car are poor when compared to other vehicles. The rear window and rear view mirror are very small. You cannot see much out of it especially when backing up. Parking in a parking lot is very problematic and dangerous because that's where people are often walking around. The side view mirrors are better than the rear view, but the driver side mirror keeps self adjusting while driving and I have to keep messing with it which makes it unreliable for use. Tesla has been awful in keeping up. Please help!
I took delivery on a 2025 Tesla Y on Oct 17. All cameras and gps stopped working on Nov 30, including the backup camera. Based on posts online, this problem is widespread but Tesla has not sent any notification. Tesla Service says the computer has to be replaced and there is a lengthy delay in getting the replacements. My appointment has been moved twice, from Dec 5 to Dec 9 and now to Jan 9. For months, many people will be driving without a backup camera in a car not designed for visibility. It’s a real safety issue and at a minimum tesla should warn current owners of the problem and stop delivering the cars that will likely be impacted. Instead, i have been unable to get any acknowledgement from Tesla that this is a known or widespread issue. They are also not making loaner cars available while waiting for the part to fix the problem. I’ve been told “it’s not a safety issue.”
Around the end of November, my 2024 Tesla Model Y's cameras stopped functioning. The sensors for blind spots and possible collision prevention stopped working. The GPS was also not working, the car believes it's been in the same spot for over a month. The lights of the car also do not work as promised, as now I have to manually turn them on on my dashboard and it goes on and off. I have been trying to get into Tesla to have this fixed but the appointment keeps getting pushed out due to the part that needs to be fixed not being in stock. I brought my car into Tesla for them to confirm that it is the part that is missing and that is the case but they couldn't fix it because they don't have the part. It's been over a month and my date was pushed out again, probably the sixth or seventh time. Tesla claims that there will be a software update that will fix this issue, however, my car is not updating and won't even accept a software update.
Tesla has told me the AP4 computer has failed in my car which has resulted in them having to replace it. I had this issue start right around Thanksgiving. I scheduled the soonest appointment available for 01/03. Not offering me a rental or anything on my brand new '24 Tesla. It has just over 1,000 miles on it and is ~1 months old now. This issue impacts: emergency braking, blind spot detection, lane keep/centering, assisting with parking, automatic high beams and wipers, cruise control/ adaptive cruise control, back camera, Vehicles built in GPS to be stuck at one location every time, has made my touchscreen not automatically brighten or dim, my auto dimming mirrors be too dim in most driving conditions, constantly fail to install system improvements and safety updates, and because my vehicle keeps trying to do this, it has caused increased battery drain as well. Components Failing - Reported as central computer. This made the vehicle turn signals and backup cameras fail as critical components. These two elements were/are completely non-functional. Additional less critical safety functions that have been disabled include blind spot monitoring and cameras, both forward and reverse emergency braking, and pedestrian alerts. My safety was put at-risk due to camera failures while driving and backing out of parking spaces. These vehicles are designed with poor and minimal visibility due to their cameras compensating for visibility shortcomings. You cannot adequately see proximity to other vehicles or pedestrians when cameras are not active. This particular impacts reversing when backing in and out of parking spots since the rear windshield is so small and poorly angled. Drivers have reported that the cameras for them are failing mid-drive leading them to not know where other vehicles were next to them and making unsafe lane changes thinking their warning systems were active. The vehicle component has NOT been inspected by anyone as there was thankfully no actually accident or incident due to the failure. Tesla has not inspected the vehicle due to NO availability for 3+ weeks and they will not consider human contact to expedite review. There are now 100s of community reports of the same defect on online communities all showing failures on same-day or within 24 hours of each other. There were no warnings prior to the failure. The symptoms simply stopped working. It required a phone call to Tesla support to even receive an accurate error code. Tesla is claiming a "short" in the main computer system of the vehicle. This occurred for myself and dozens of others all within several hours of each other in the early morning hours of Friday November 29th 2024. Suspicion is that that this was due to a software update from Tesla.
The contact owns a 2024 Tesla Model Y. The contact stated while driving at an undisclosed speed with Full Self-Driving Assist Mode activated, the vehicle inadvertently veered off the road, drove over a curb and came to a complete stop. No warning lights were illuminated. The contact stated that the vehicle then operated as needed; however, the failure recurred several times while driving. The manufacturer was not notified of the failure. The vehicle was not diagnosed or repaired. The failure mileage was approximately 3,000.
Cameras not working with latest Software update on November 26th. without even inspecting the car in-person, Tesla service agent told need new AP4 computer on 2024 model Y which is 1 month old . I think Tesla knows about this issue and there is lot of wait time for getting the AP4 computer. Tesla service advisor states - car is totally safe to drive with just mirrors till appointment in few weeks. As per NHTSA, if a car is not having a backup camera, that is a safety violation and there are lots of Tesla vehicles driving without backup cameras. No backup camera, navigation, battery life dying faster. It seems like a liability to me to drive this way and very negligent to me.
All the cameras for the Tesla Model Y has stopped working, without this lots of features stopped working including rear camera which is needed to reverse to charge the vehicle and anyware to park. All the collision warning systems including front and side stopped working. Wipers don't activate and figured to change to manual. Headlights always stay in highbeam. Looks like there are known issues and Tesla claims they don't have hardware to replace making us to not use the car for almost 30 days.
At night, on paved and well-painted rural roads, where there is no other sources of light, the pillar cameras incorrectly warn they are blocked or blinded. At the same time the Lane Departure Warning and Lane Keeping Assistance stop functioning without warning. On the image below I let the car drift over the line. As I crossed the line, the display should have highlighted the line in blue and there should have been a flashing warning at the top of the screen and an audible warning. None of those occurred. Since lane keeping assistance was enabled. It should have also steered the car back into the lane. I can regularly reproduce the issue. We don't understand why they are using the pillar cameras for this and not the forward cameras that can take advantage of the headlights. It is unsafe, especially since I am not warned of the system is not available.
The rear (reverse) camera and side cameras (ie lane change) do not function. The navigation does not function. The console touchscreen does not adapt to environment light level for safe viewing.
All safety sensors appear to be out of service. Navigation is stuck at one location and is not working. All cameras, including backup, are out of operation (only shows dark screen). Back up sensor does not work. This issue has been ongoing for 1 month on a brand new vehicle. Tesla service center has no ETA on resolution. Service appointment has already been pushed back to next year (Jan 2025).
On Nov 23, 2023, I was reversing car from my driveway in the morning. Car started giving warnings with issue on all its cameras. While reversing car, it will give error that rear view camera not available. While giving left/ right turn signal, it is showing blank screen for that view. All other safety features like blind spot detection, lane assist are not working at all. I also started getting error message that emergency brake is not available. I have been contacting them for immediate repair/ provide loaner car from same date (11/23/2023). Earliest appointment date given is Dec 11, 2023. This is not acceptable at all. It is very dangerous to drive the car without those safety futures. I am also concerned on any other issues which it has not reported. I don't feel safe to drive the car.
Back in late November 2024, Tesla pushed an Over-The-Air (OTA) update to my 2024 Tesla Model Y. Immediately after pushing this OTA, all of my car's cameras stopped working (INCLUDING my rear or "backup" camera). This is in violation of federal law as all new vehicles are required to have a backup camera. Additional safety-critical items were also impacted: the rear-view and both of the side mirrors remain tinted to the darkest level, reducing my visibility. The side mirrors are also tilted in the "downward" position, making lane changes incredibly difficult. Since the front-facing and side-facing cameras do not work the vehicle's forward and side collision detection systems are inoperable. The vehicle suffers from additional failures (GPS navigation, Maps, Autopilot, among many others). I have presented these issues to Tesla and they have continously pushed out my repair date. I have also asked for a loaner vehicle due to the failure of the car's safety systems (some of which are federally mandated!!). Tesla has ignored my pleas for a loaner vehicle and has also remained silent when I have repeatedly asked to return the vehicle under Washington State lemon laws. My safety and the safety of others have been put at grave risk due to the backup camera and collision detection cameras not working. Tesla has NOT inspected the vehicle as they continuously push my service date further out. No obvious warning lamps, messages, or other symptoms appear. The user doesn't know these federally-mandated safety features are inoperable until they put the car in "reverse" or attempt a lane change.

I scheduled the soonest appointment available for 12/13 however Tesla contacted me and moved my appointment because they stated that the part was unavailable at this time and should be delivered by 01/20/25. Not offering me a rental or anything on my brand new '24 Tesla. It has just over 2500 miles on it and is ~2 months old now. Additionally, the vehicle came to a stop on the highway when my wife and I were in the car on the night of [XXX], forcing us to perform an emergency pullover and abandon the car 22 miles north of Rochester, MN overnight. This has increased the safety risk of my vehicle tremendously... Tesla has not seemed to publicly acknowledge the issue yet either. One day the cameras, gps, and sensors just stopped working and haven’t worked since. Been 4 weeks.
Auto high beams: They come far too soon or don't dim when when following other vehicles, they do not dim soon enough when meeting cars over rolling hills. They do not dim for oncoming vehicles on a slight bend in the road or on intersecting streets. They also do not dim when in town on lit streets. I've tried to turn them off and manually operate the lights. You are unable to disable the auto high beams for more than a brief period when FSD is available (enabled or disabled). Any action (turning on legacy cruise control, Auto-steer beta, FSD, disengaging and reengaging FSD, shifting the car out of part, etc) turns the auto high beams on regardless of whether or not I disabled them previously.
My Tesla Model Y experienced a complete computer failure, rendering all active safety systems inoperable. This included backup cameras, speedometer display brightness, and collision avoidance system. After receiving assistance from Tesla service, the issue was diagnosed as a shorted computer. A computer replacement was performed, resolving the problem. I am submitting this case as a reference for potential investigations.
I had my computer replaced (AP4) on the Model Y. After a software update around Nov 13, my computer glitched. The car was drivable, but no functionality (cameras, navigation. Etc). After the computer was replaced it seems fine. Just wanted to report after the fact. Thanks
I noticed that now Tesla uses windshield mounted cameras for everything in front of car including FSD and forward collision. My model y was made in fremont, California. Problem is windshield glass is not cleaned before assembly to car. Either not a prescribed step by Tesla during assembly or overlooked by assembly workers. I noticed after I picked up my new model y from Tesla that there is haze and layer of residue on windshield glass. As if glass are sitting in bulk in factory and assembled to car without cleaning. I was able to clean windshield inside with glass cleaner and microfiber cloth. Problem is windshield haze and Layer of residue cannot come out of front facing camera module. This is causing all functions to degrade that rely on these windshield mounted cameras. I get random "one of cameras occulted" error during Full self driving FSD. Car cuts out during summon. Haze can be seen on app cameras during summon, while rear camera and fender cameras show clear view. This will get worse with winter now as fog also sets on windshield. Cleaning windshield glass behind camera module requires professional disassembly. Maybe it only affects model y made in fremont, CA or more
All cameras are not working anymore including back up camera. Tesla acknowledged the issue and stated that the Computer needs to be replaced and a lot of cars have similar issues.
Subject: Urgent Safety Concern and Request for Immediate Resolution – Cracked Sunroof on Tesla Model Y 2024 To Whom It May Concern, My name is [XXX] , a United States citizen and resident of Florida. I am writing to formally express my deep concern and dissatisfaction regarding a significant safety issue with my Tesla Model Y 2024, which I purchased directly through Tesla’s official website. To date, my vehicle has never been involved in any accident and has been consistently stored in a secure garage, ensuring that it remains free from external damage. Issue Description: Recently, I discovered that the glassroof of my Tesla Model Y has developed noticeable cracks. This issue is perplexing and unacceptable, especially considering the vehicle has not been subjected to any conditions that could cause such damage. The sudden appearance of these cracks suggests a potential manufacturing defect or poor assembly quality, both of which are serious concerns for vehicle safety and integrity. Previous Communication: Upon discovering the issue, I promptly contacted Tesla Service to report the problem and seek assistance. Unfortunately, my claim was denied without a satisfactory explanation or resolution. This denial not only exacerbates my frustration but also raises significant concerns about the quality control and customer support standards upheld by Tesla. Safety Concerns: A cracked roof compromises the structural integrity of the vehicle and poses substantial safety risks to me and my passengers. glassroof malfunctions can lead to unexpected failures, potentially resulting in accidents or injury. Additionally, the aesthetic damage detracts from the overall value and appeal of the vehicle. Request for Immediate Action: Given the severity of this issue, I respectfully request the following: Immediate Investigation: A thorough investigation into the cause of the glassroof cracks, including a detailed assessment of potential manufacturing defects or assembly INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORM
All safety features and cameras on this vehicle are currently disabled. Specifically, the following functions are not operational: Forward and parking collision warnings, Blind spot warning, Lane departure and lane-keeping warnings, Rear cross-traffic warning, Self-driving capability, Adaptive cruise control, Navigation system, Location tracking, Sentry mode, Premium connectivity features, Reverse camera, Front and side cameras. Additionally: the headlights remain on continuously during daylight hours. The interior control panel screen and the side and rear mirror displays are stuck in night mode.
While driving between 50-55 mph the Tesla applied emergency breaking even though there was nothing in front of us and no risk of accident. The car went from 55 to about 5-10mph in the matter of a second. We are lucky no one was directly behind us or it would have led to a high speed rear end accident.
All of the car's cameras have gone dead since November 2024. The car has been collecting water in the truck area and inside panel of the car by the door frame.

The seat belt in the back right seat stopped latching after a few months of use. It’s where we buckle my child on his booster seat so I was very concerned that it failed
What component or system failed or malfunctioned, and is it available for inspection upon request? The "Actually Smart Summon" failed. It is available for inspection. How was your safety or the safety of others put at risk? The "Actually Smart Summon" function did not detect the post next to my vehicle as it was exiting the parking space. It immediately turn right into the post instead of pulling straight out first. I had very little reaction time. If there had been a person in the position of the post, this could have caused physical harm. My vehicle sustained damage to the front passenger door, its side mirror, and to the PPF material over these components. Has the problem been reproduced or confirmed by a dealer or independent service center? I don't know Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? When opening the feature on the app, it states: "Warning: Monitor vehicle's surroundings for people, animals, or unpredictable movement."
None of the cameras work in the Tesla. Unable to use any safety features. No reverse camera. Any adaptive cruise control, safety features are all disabled. Two months to get it repaired and serviced.
#Issue Description I experienced an issue with Tesla’s Full Self-Driving (FSD) system when a trailer hitch was attached to my vehicle. From what I observed, the FSD system appeared to detect the trailer hitch as an object in close proximity to the vehicle. This caused the vehicle to accelerate rapidly to the FSD maximum speed of 85 MPH, regardless of the road conditions or posted speed limits. This issue occurred when the lane markings were visible, but the hitch obstructed the rear camera’s view. On the FSD display, this obstruction was represented as a persistent “cloud” following the vehicle. I attempted to adjust the maximum speed limit manually to mitigate the acceleration, but this did not prevent the behavior. Each time I engaged FSD, the car would accelerate rapidly, even in situations where such high speeds were not appropriate for the road conditions. # Steps Taken to Resolve the Issue I covered the rear camera, which allowed FSD to function normally and prevented the rapid acceleration. The “cloud” visualization on the FSD display also disappeared once the camera was covered. # Trailer Mode Tesla offers a “Trailer Mode,” but I did not enable it since I was only using a hitch without towing a trailer. My understanding is that FSD does not operate in Trailer Mode. # Reference: This issue has been reported by other Tesla owners, as discussed here: [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
In Auto Carwash while I was in vehicle it shifted from Neutral to Park causing the car behind me to hit my vehicle
I have had this car since May 2024 and the current mileage on the vehicle is 7,130 miles. In the past few weeks, I have had to completely replace both rear OEM aluminum alloy wheels due to cracks caused by normal road driving, which caused both rear tires to go flat. Per Tesla, this costly replacement is not covered under their warranty. I am now learning that this is a common issue with these particular wheels on this particular model (Model Y Performance). It appears the aluminum alloy wheels are simply not sturdy enough to bear the weight of the vehicle and the required tires simply do not provide sufficient cushioning to prevent wheel cracks resulting from normal wear and tear/road driving. Many customers have had to replace the OEM wheels with aftermarket wheels made of forged steel. These customers report zero issues with the much stronger wheels. This appears to be a product design or manufacturing problem, which is creating a significant consumer/public health risk.
With beta self driving engaged, I turned south on [XXX] intending to turn west on [XXX]. At [XXX], there are 2 right turn lanes. I approached the intersection turning from the left lane. Rather than turn right, the car continued straight. I was able to manually override the system and luckily brought the car under control; otherwise, I am sure I would have ended up in a very bad spot. Later the same day beta did not recognize a turn lane into my community but rather proceeded straight. I have since disabled the beta FSD. I am responding to today's CNN article of your pending investigation. I cannot respond to your other questions above. Thank you! While driving on the highway got the warning on the control screen with red light warning stating "Pull over and stop the vehicle" and the car automatically started slowing down. Another warning popped up stating “Power Reduced - Exiting and re-entering vehicle may restore operation”. Has a yellow icon of a turtle. Felt very dangerous on a highway and immediately pulled over to the shoulder.
My car slammed on the brakes on the highway this morning at 75 mph. A box truck was following me but thankfully he didn't hit me. I was in what Tesla calls Traffic Aware Cruise Control (TACC) at the time and quickly brought the car out of TACC and hit the accelerator to get me back up to speed so I didn't pose a danger to other drivers on the highway. I dont know exactly how much my car slowed, but it was enough to throw me forward in my seat. There were no vehicles in front of me at the time. This was a classic example of "phantom braking", which is talked about a lot in the Tesla community. This is my 2nd report of phantom braking to the NHTSA.
In a 90 mile drive I experience phantom breaking 4 different times. No car was in front of me, in fact nothing at all was in front of me when my car slammed on the brakes 4 separate times. One time there was a car somewhat close behind me. I also twice experienced audible warnings and flashing alarms to take control immediately of the car when my hands were on the wheel and I was already in control of the car, it was alarming and shocking to get these alarms while already in control and driving. I believe this car, as currently configured, can pose a danger to other drivers on the road.
The car randomly restarts while driving and some functionalities doesn’t work like turn signal, ac, radio, headlight. It could lead to the accident. I reported to Tesla and they said it is fixed with reset. It is still happening.
I have had multiple incidents of “phantom braking” when using traffic aware cruise control and auto steer. It usually happens on one lane state highways with opposite direction traffic, sometimes with turns or hills. The vehicle has slammed on the brakes without warning, sometimes the warning tone also sounds. Each time there was absolutely no danger, nothing abnormal. On a couple occasions there was traffic behind me, and it must have scared them. They probably thought it was a hard brake check. If I wasn’t able to quickly over ride the braking with the accelerator it very easily could have caused an accident. On two occasions my pregnant wife was in the car and it scared her very much. I am personally afraid to use it now, knowing it’s going to slam on the brakes is nerve wracking and UNSAFE. Tesla does not seem to care about addressing the issue, rather putting all eggs in the “full self driving” basket instead of improving this very obvious safety defect. From reading online forums it seems to affect just about everyone. I’m hoping their engineers address it and they release a software update correcting it, but as mentioned above it doesn’t seem to be on their list of priorities; the problem has been happening for several years now and has resulted in at least one known fatality. Hopefully the NHTSA can light a fire under them and have this scary safety defect addressed.
The contact owns a 2024 Tesla Model Y. The contact stated that while at an undisclosed speed, the vehicle had accelerated independently. No warning lights were illuminated. On another occasion, while exiting a parking lot the vehicle unintendedly accelerated causing it to crash into a parked vehicle. The auto wipers come on multiple times a drive without water being on the windshield. This has been dangerous because the wipers will smear material across the windshield at the wrong times creating a visibility hazard. It almost caused a wreck where it activated smearing the windshield while the sun was directly ahead. Tesla’s response was the auto wipers are in Beta mode and I should turn them off to keep them from randomly coming on. This is a challenge since you have to use the center screen to find the wipers and activate them causing one to look away from the roadway.
I was in a parking lot on [XXX] in Mamaroneck, New York waiting in line to get out at approximately [XXX]. Many cars were ahead of me due to the school drop-off rush, with parents and kids lined up in the parking lot waiting to leave. While I was waiting, the Tesla unexpectedly accelerated on its own, but I managed to hit the brakes just in time to avoid colliding with the car in front of me. A few seconds later, my car suddenly accelerated again without any input from me. I tried to steer away from the cars in front while repeatedly pressing the brakes, but they weren’t working. All I could think about was avoiding hitting another car or person, so I swerved the car toward the building in the parking lot. The impact was hard, and the car finally stopped, but the airbags didn’t deploy. I’m deeply shaken by the incident; I could have seriously injured someone or even worse. When I got home, I discovered that this issue is a known problem with Tesla, yet they’re placing the blame on drivers. Having never been in a car accident before, this experience has been overwhelming. As a mother and attorney, I believe it’s crucial to examine this situation more closely. Tesla controls the data, and it’s clear that responsible drivers wouldn’t suddenly veer into a building in a crowded parking lot without cause. I urge you to act on this life threatening defective car and investigate this problem before it is too late. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
