The contact stated that failure was caused by brake fluid leaking onto a faulty brake pressor sensor switch, also called a speed control deactivation switch under the hood, and caused the switch to corrode, causing the fire. The contact's husband was then contacted by his co-workers via walkie-talkie to inform him the vehicle was on fire. When the contact arrived at the scene of the fire, nine other co-workers and himself attempted to extinguish the fire with water and extinguishers however wires were burnt on the front part of the vehicle and they were unsuccessful. The Fire Department was on scene and extinguished the fire. The origin of the fire was unknown, but the fire department did state it had to do with something electric. The fire took place at [XXX] . The contact stated that while driving at various speeds, the front passenger’s seat belt loosened independently, and the front driver’s side seat belt occasionally loosened independently. The local dealer informed the contact that parts for recall repairs were not on backorder. The vehicle was not repaired. The manufacturer was made aware of the failure. The vehicle had been engulfed in flames and the fire department had been called and extinguished the fire using water. The contact stated that a fire report was issued. While we were driving the truck started shaking bad and smoke comes out of the tire
WE PURCHASED OUR 2001 F150 SUPERCREW USED. I AM A MECHANIC, SO THE FIRST THING I DO WHEN BUYING A USED VEHICLE IS INSPECT IT THOROUGHLY. THERE WERE A FEW MINOR ISSUES, BUT ONE STOOD OUT AS A MAJOR SAFETY HAZARD. UPON FURTHER RESEARCH, IT SEEMS TO BE EXTREMELY COMMON TO ALL 1997-2003 F-150S. SYMPTOMS - THIS ISSUE MAINLY AFFECTS THE REAR LEFT AND RIGHT BRAKE LIGHTS, TURN SIGNALS, AND HAZARD LIGHTS. THESE LIGHTS, PARTICULARLY BRAKE LIGHTS, CAN CEASE TO FUNCTION ENTIRELY, INTERMITTENTLY AND UNKNOWN TO THE DRIVER. CAUSE - THERE IS A WIRE HARNESS WHICH CONNECTS TO THE "MULTI - FUNCTION SWITCH". THIS WIRE HARNESS IS PULLED WHEN THE STEERING WHEEL IS TILTED DOWN. AS A RESULT, THE CONNECTOR OR INDIVIDUAL TERMINALS CAN BECOME LOOSE OR DISCONNECTED. THIS PROBLEM IS MOST PREVALENT WITH THE STEERING WHEEL IN THE DOWN POSITION, BUT ONCE LOOSE CAN HAPPEN AT ANY POSITION AND ANY DRIVING SITUATION. SOLUTION - REPAIR OR REPLACE WITH MOTORCRAFT PART# WPT607, AND ENSURE THE HARNESS HAS ENOUGH SLACK TO ALLOW STEERING WHEEL TILT WITHOUT TENSION ON THE HARNESS. IN OUR CASE, I REPLACED THE CONNECTOR AND PULLED THE WIRES ENOUGH TO ALLOW SLACK FOR STEERING WHEEL TILT. THERE ARE NUMEROUS REPORTS OF THIS ISSUE ONLINE, AND IT IS WELL DOCUMENTED BY F150 OWNERS. I HAVE PICTURES BUT THE FILE SIZES ARE OVER 5MB.
TL* THE CONTACT OWNS A 2001 FORD F-150. THE CONTACT STATED THEY DROVE THEIR VEHICLE TO THE STORE AND THEN BACK TO THEIR RESIDENCE. A FEW MINUTES LATER THE CONTACTS NEPHEW CAME INTO THE RESIDENCE AND STATED THEY SAW SMOKE COMING FROM THEIR VEHICLE. WHEN THE CONTACT WENT BACK OUTSIDE THEY SAW SMOKE AND FIRE COMING FROM UNDERNEATH THE HOOD. THE CONTACT WAS ABLE TO LIFT THE FRONT HOOD OF THE VEHICLE AND WITH A WATER GARDEN HOSE WAS ABLE TO EXTINGUISH THE FIRE. NO ONE WAS INJURED, THERE WAS NO FIRE DEPARTMENT REPORT FILED. THE VEHICLE WAS NOT DIAGNOSED OR REPAIRED. THE MANUFACTURER WAS NOT MADE AWARE OF THE FAILURE. THE FAILURE MILEAGE WAS 160,000.
CLUTCH FAN BELT
TL* THE CONTACT'S BOYFRIEND OWNS A 2001 FORD F-150. THE CONTACT STATED THAT WHILE DRIVING AT VARIOUS SPEEDS, SHE COULD HEAR AN ABNORMAL GRINDING SOUND COMING FROM THE VEHICLE, WITHOUT WARNING. UPON INVESTIGATION, THE CONTACT DISCOVERED THAT ONE OF THE BRAKE BRACKETS HAD LOOSENED AND DAMAGED THE REAR DRIVER'S SIDE TIRE. THE CONTACT TOOK THE VEHICLE TO AN INDEPENDENT MECHANIC WHERE THE BRAKE PADS WERE REPLACED HOWEVER, THE FAILURE RECURRED SOON AFTER THE REPAIR. THE CONTACT TOOK THE VEHICLE TO ANOTHER INDEPENDENT MECHANIC WHERE THE FAILURE WAS LINKED TO NHTSA CAMPAIGN NUMBER: 01V095000 (SERVICE BRAKES, HYDRAULIC) AND INFORMED HER TO CONTACT THE MANUFACTURER. THE CONTACT TOOK THE VEHICLE TO MAC HAIK FORD (10333 KATY FWY, HOUSTON, TX 77024) AND WAS INFORMED THAT THE VEHICLE WAS NOT UNDER RECALL, AND PROVIDED THE CONTACT WITH AN ESTIMATE FOR THE REPAIR. THE MANUFACTURER WAS NOT NOTIFIED OF THE FAILURE. THE VEHICLE WAS NOT REPAIRED. THE FAILURE MILEAGE WAS UNKNOWN.
TL* THE CONTACT OWNS A 2001 FORD F-150. THE CONTACT STATED THAT AFTER PULLING INTO THE DRIVEWAY AT THE RESIDENCE, THE SPARE TIRE DETACHED FROM THE VEHICLE. THE CONTACT BECAME AWARE THAT THE SPARE TIRE CABLE WAS FRACTURED. THE VEHICLE WAS NOT TAKEN TO A DEALER TO BE INSPECTED. THE MANUFACTURER WAS CONTACTED AND INFORMED OF THE FAILURE. WHILE GIVING A CUSTOMER A QUOTE (WORK) MY TRUCK STARTED SMOKING. I OPENED THE DRIVER SIDE DOOR FLAMES HAD ALREADY TAKEN OVER, THERE WAS NO SAVING ANYTHING. SO I THEN CALLED 911 & TRIED PUTTING IT OUT WITH WHAT RESOURCES WE DID HAVE. WE HAD TO MANUALLY UNHOOK MY LAWN TRAILER FROM TRUCK TO PREVENT FROM LOOSING MY ENTIRE LIVELIHOOD.
FRAME BENT DUE TO UNSEEN CORROSION. THIS HAPPEN WHEN TRUCK WAS ON A LIFT FOR REPAIRS TO THE TRANSMISSION LINES. MECHANIC HAD NO WAY OF KNOWING. VEHICLE SPENT HALF ITS LIFE IN ARIZONA WHERE NO SALT IS USED ON THE ROADS
MY DRIVERS SEAT BELT BUCKLE DOES NOT WORK PROPERLY.
BRAKES COMPLETELY FAILED. NO BRAKE PEDAL. CRUISE CONTROL RELEASE ON MASTER CYLINDER BROKE OPEN. LUCKILY IT WAS IN A PARKING LOT, NOT ON THE HIGHWAY. VEHICLE HAD NO BRAKES WHATSO EVER.

I OWN A 2001 F150 SUPER CAB 44 THE FORD MOTOR COMPANY HAS ALREADY ADMITTED THAT THE PROBLEM WAS THEIR FAULT SOMETHING ABOUT FOR SOME REASON THEY LEFT METAL SHAVINGS BETWEEN THE HEAD OF THE MOTOR AND THE BLOCK OF THE MOTOR CAUSING IT TO LEAK OIL REALLY REALLY BAD ONTO THE STARTER AND THE EXHAUST MANIFOLD AND IT JUST POURS OIL AS FAST AS YOU CAN POUR IT IN WHICH IS A HIGH PROBABILITY OF THE EXHAUST MANIFOLD TO IGNITE THE OIL AND BURST INTO FLAMES WHY THEY HAVEN'T ALREADY DONE A RECALL IS BEYOND ME THEY MADE A UPGRADED HEAD GASKET THAT THEY WILL SELL YOU AND YOU CAN FIX THEIR MISTAKE ON YOUR OWN NICKEL BUT I DON'T HAVE OVER 3000 DOLLARS TO PULL THE ENGINE AND FIX THE PROBLEM ITS A 5.4 ENGINE AND EVERY ONE I HAVE SEEN AROUND THE SAME YEAR MODEL HAS THE EXACT SAME OIL LEAK THAT COULD BE VERY DANGEROUS NOT TO MENTION IT HAS ALREADY MESSED UP THE INTEGRITY OF MY ENGINE
DEAR NHTSA: RE: VIN# [XXX] MY FORD 150 EXTENDED CAB BACK DOORS LATCHES HAVE FAILED. THE TWO BACK DOORS DO NOT OPEN. AT FIRST, I THOUGHT THE RUBBER THAT RUNS ALONG SIDE THE TRIM WAS CAUGHT, BUT WHEN THE SECOND DOOR STOPPED OPENING I TOOK IT TO A FORD DEALERSHIP AND HE SAID THERE ARE TWO LATCHES ON EACH DOOR , A BOTTOM LATCH AND A TOP LATCH. BOTH TOP LATCHES HAVE FAILED AND NOW I CANNOT OPEN THE TWO BACK DOORS TO MY FORD TRUCK. THE FIRST DOOR(DRIVER SIDE DOOR) WENT OUT IN NOVEMBER OF 2019, AND THE SECOND DOOR STOPPED OPENING LAST WEEK. TL* THE CONTACT OWNS A 2001 FORD F-150. WHILE DRIVING 48 MPH, WHILE THE BRAKE PEDAL WAS DEPRESSED, THE VEHICLE FAILED TO COME TO A COMPLETE STOP. AS A RESULT, THE VEHICLE CRASHED INTO AN 18-WHEELER. THE AIR BAGS DEPLOYED. THE VEHICLE WAS TOWED TO THE CONTACT'S RESIDENCE. A POLICE REPORT WAS FILED AND THE CONTACT WAS INFORMED THAT THE REAR BRAKES APPEARED TO HAVE FAILED. THE CONTACT SUSTAINED A LACERATION ON THE LEFT ARM, NECK AND BACK PAIN, AND A BRUISE ON THE CHEST. THE CONTACT DENIED MEDICAL ATTENTION, BUT LATER WENT TO THE HOSPITAL. THE VEHICLE WAS NOT TAKEN TO A DEALER OR INDEPENDENT MECHANIC FOR DIAGNOSTIC TESTING OR REPAIRS. THE MANUFACTURER WAS NOT MADE AWARE OF THE FAILURE. THE FAILURE MILEAGE WAS APPROXIMATELY 250,000.
TRUCK EXPLODED WHILE PARKED IN THE DRIVEWAY.
THE DRIVER SEAT BELT WILL UNLATCH ON IT'S OWN WHILE THE CAR IS IN MOTION. DRIVER REATTACHED THE BUCKLE. DRIVER PULLED ON BUCKLE TO MAKE SURE IT WAS SECURE. SEAT BELT UNLATCHED 2 MORE TIMES WHILE DRIVING.
BRAKE SWITCH CAUGHT ON FIRE, BURN MASTER CYLINDER RESERVOIR AND WIRES
MY VEHICLE HAS AN AUTOMATIC TRANSMISSION. I PULLED OUT OF MY DRIVEWAY AND SHIFTED INTO DRIVE AND THE GEAR SHIFTER WENT "FLOPPY," BECAME EXCESSIVELY LOOSE AND WOULD NOT SHIFT INTO ANY OTHER GEAR POSITION. I HAD TO SHUT THE ENGINE OFF AND SET THE EMERGENCY BRAKE TO KEEP THE VEHICLE IN PLACE RIGHT THERE IN THE MIDDLE OF THE STREET. IT TURNS OUT TWO 27MM TORX BOLTS HAD COME LOOSE ON THE STEERING COLUMN SHIFTER ASSEMBLY INSIDE THE DASHBOARD (A NUT HOLDING ONE OF THE BOLTS HAD ACTUALLY BROKEN INTO TWO PIECES). SEE ATTACHED PICTURES. AS I RESEARCHED THIS PROBLEM, I LEARNED THAT HUNDREDS, PERHAPS THOUSANDS HAD REPORTED THE SAME ISSUE (THAT AND BUSHINGS BREAKING OR SLIPPING OUT ON THE STEERING COLUMN CAUSING A SIMILAR ISSUE), ON F-150'S, F-250'S, F-350'S, WINDSTAR VANS, AND OTHER FORD VEHICLES. THIS HAD THE POTENTIAL TO BE A TRAGIC PROBLEM. C'MON, FORD, TAKE SOME OWNERSHIP AND DO RIGHT BY YOUR CUSTOMERS HERE--FIX THIS PROBLEM UNDER RECALL!

THE CONTACT WAS NOT AWARE OR NOTIFIED OF A SPEED CONTROL SYSTEM MODIFICATION FROM JANUARY 27, 2005 AND NHTSA CAMPAIGN NUMBER: 11V385000 (FUEL SYSTEM, GASOLINE) AS OF JULY 29, 2011. THE DEALER WAS NOT NOTIFIED. THE MANUFACTURER WAS NOTIFIED OF THE FAILURE. THE APPROXIMATE FAILURE MILEAGE WAS 275,000.
MY 2001 FORD F150 HARLEY DAVIDSON TRUCK HAS ACCELERATED ON IT'S OWN WHILE I DROVE INTO A PARKING SPOT. FORTUNATELY THERE WAS A PARKING CEMENT PIECE THAT STOPPED MY TRUCK FROM HITTING A WALL. THERE SEEMS TO BE NO RECALLS ON MY VEHICLE FOR THIS ISSUE BASED ON THE VIN. IT DOESN'T MAKE SENSE. I BELIEVE THAT THIS IS A SAFETY HAZARD.

WHILE DRIVING ON THE FREEWAY HERE IN SAN DIEGO, CA. ON DEC. 31, 2018 AT 11:15 AM, AT 65 MPH I NOTICED A CAR IN FRONT OF ME SWERVE AND BEFORE A COULD DODGE THE OBJECT I HIT IT WITH MY LEFT FRONT TIRE. AFTER MANAGING TO GET OVER TO THE RIGHT SIDE OF THE SHOULDER OF THE ROAD IN HEAVY TRAFFIC I OBSERVED MY TIRE WAS FLAT AND DESTROYED BEYOND REPAIR. AFTER REPLACING IT WITH THE SPARE IN THE RAIN. I RETURNED HOME TO CHECK FOR ANY DAMAGE TO MY TRUCK, BUT FOUND NONE. AFTER A CALL THE THE PEPBOYS STORE WHERE I PURCHASED THEM ON APR. 30, 2013, AS WELL AS THE ROAD HAZARD SERVICE PLAN, I LEARNED THEY DO NOT CARRY THESE TIRES ANY MORE. I WAS OFFERED A DIFFERENT BRAND TIRE AS A REPLACEMENT. CAN YOU IMAGINE DRIVING YOUR PRIDE AND JOY WITH THREE TIRES OF ONE BRAND AND ONE OF A DIFFERENT BRAND. UNACCEPTABLE!! AS OF NOW READING THE OTHER MANY COMPLAINTS ABOUT THESE TIRES I CAN ONLY ACCEPT THEM REPLACING ALL THE TIRES UNDER WARRANTY.

THE DEALER MENTIONED AN UNKNOWN RECALL RELATED TO THE FAILURE THAT WAS CLOSED. THE VEHICLE WAS NOT REPAIRED. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE, BUT DECLINED TO ASSIST SINCE THE CONTACT RECEIVED SEVERAL RECALL NOTICES. THE CONTACT STATED THAT THE VEHICLE WAS INCLUDED IN NHTSA CAMPAIGN NUMBER: 05V388000 (VEHICLE SPEED CONTROL). THE FAILURE MILEAGE WAS 160,000.
FRONT PASSENGER SEAT BELT WILL NOT LOCK. WAS ON A TRIP, ARRIVED AT DESTINATION, WENT TO LEAVE AND THE FRONT PASSENGER SEAT BELT WOULD NOT BUCKLE. STILL WON'T BUCKLE. I AM CONCERNED, WHAT IF THE DRIVERS SIDE BUCKLE FAILS WHILE I'M DRIVING IT? I DID READ OF SOME OTHERS FAILING.
ON 9/4/2018 I BOUGHT 4 BRAND NEW DESTINATION LE2 P-METRIC OWL P265/75R16 TIRES WITH A 60000 WARRANTY FROM FIRESTONE IN HESPERIA CA. MY DAUGHTER JUST BOUGHT THE TRUCK AND I WANTED HER TO BE SAFE WITH NEW TIRES.ON NOVEMBER 14, 2018 ON HER WAY TO WORK AT 5:00 A.M SHE HAD A TERRIBLE ROLL OVER. SHE IS CURRENTLY IN ICU. SHE JUST HAD A CRANIOPLASTY 15 DAYS AGO. SHE HAS A SEVERE TBI. SHE HASN'T WALKED SINCE THE DAY OF ROLLOVER. SHE WAS ON LIFE SUPPORT. STILL ON TRACHEOSTOMY. SHE WILL BE DISABLED ALL IF HER LIFE. SHE IS 22 YEARS OLD. SHE WAS DRIVING APPROX. 55 MPH. SHE WAS OUT OF CONTROL WHEN ALL THREE WITNESSES SAW HER COMING AT THEM. THIS HAPPENED ON SUMMIT VALLEY RD IN HESPERIA CA. IT IS ONE LANE EACH DIRECTION. SHE WAS APPROACHING TH TOP OF A HILL. WHEN SHE GOT OVER THE HILL SHE WAS OUT OF CONTROL AND THE VEHICLE ROLLED AND SHE WAS EJECTED. SHE WAS FOUND UNDER THE TRUCK. THE TIRES HAD LESS THAN 5,000 MILES ON THEM. PLEASE DON'T BUY THESE TIRES. I AM STILL NOT SURE IF SHE WILL SURVIVE THIS. I AM SO HEARTBROKEN I CAN BARELY MOVE.

FUEL TANK / FILLER HOSE LEAKED AND FUEL RAN ONTO THE GROUND WHILE FILLING THE TANK WITH GAS, DUE TO A HOLE CAUSED BY SIGNIFICANT RUSTING
DRIVER STARTED TRUCK ,PUT IT IN REVERSE AND EXPERIENCED UNINTENDED ACCELERATION. WITNESSES OBSERVED REAR TIRES SPINNING, HEARD ENGINE REVVING UP AND DOWN AND BRAKE LIGHTS ON. TRUCK STRUCK A STONE WALL, BOUNCED OFF AND CONTINUED TO ACCELERATE UNTIL IT HIT ANOTHER PARKED CAR, DESTROYING BOTH VEHICLES. DRIVER TRANSPORTED TO A LOCAL HOSPITAL. INCIDENT OCCURRED AT THE CITY HALL IN MIDDLESBORO, KENTUCKY. REPORT TAKEN BY THE MIDDLESBORO POLICE DEPARTMENT.
