This page combines three types of NHTSA data: recall campaigns (official manufacturer or government actions), owner complaints (unverified consumer reports), and crash test ratings (where available). A vehicle with many complaints is not necessarily less reliable — complaint volume correlates with sales volume and vehicle age. Recalls indicate identified defects, not overall quality. To compare this model year with others, use the year navigation in the sidebar or return to the model overview page.

Safety Features
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2020-2021 Avalon, Avalon Hybrid, Corolla, Highlander, Highlander Hybrid, RAV4, RAV4 Hybrid, Lexus ES350, Lexus RX350, Lexus RX450H, 2021 Sienna Hybrid, Lexus ES250, 2020-2022 Camry, Camry Hybrid, and ES300H vehicles. A short circuit may develop in the Occupant Classification System (OCS) sensor, preventing the front passenger air bag from deploying.
Remedy Status
Dealers will inspect and replace the sensor as necessary, free of charge. Interim letters notifying owners of the safety risk were mailed in February 2024. Some final remedy owner letters were mailed October 31, 2024. Letters will continue to be mailed in phases through January 2026. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is 23TB15/23TA15 Lexus 23LB03/23LA03.
The hatchback window shattered without any known cause. It appeared to have been hit from inside the vehicle but no one was in or around the vehicle when it occurred. It was 82 degrees outside and the vehicle was parked in the shade. Lexus told me there were no known issues regarding the window on other Lexus vehicles.
I am submitting this correspondence to formally report a serious safety incident involving my 2020 Lexus RX 350 and to request a thorough investigation. On Saturday, February 7, while attempting to park the vehicle, I experienced a sudden loss of control, resulting in the vehicle accelerating forward and colliding with a metal gate. The exact cause of this event is unknown. During the incident, the vehicle’s pre-collision and automatic braking system failed to engage despite an obstruction being present. Additionally, the airbags did not deploy at the time of impact. A minor child was present in the vehicle, properly secured in a child's safety seat. While no injuries occurred, the vehicle sustained extensive damage and was rendered inoperable. I would also like to note that this vehicle was subject to an airbag recall on November 29, 2025, which was serviced prior to this incident. Despite completing the recall service, the airbags failed to deploy during the accident. After the vehicle was towed, the airbags unexpectedly deployed while it was being lifted onto the tow truck using a crane, further raising concerns regarding the reliability and safety of the airbag system.
I own a 2020 Lexus RX 450h with approximately 129,803 miles. The check engine light came on and the vehicle stored diagnostic trouble code P0A2D, which indicates a failure of the drive motor/temperature sensor circuit that is integrated into the transmission/hybrid motor assembly. According to the Lexus dealer’s diagnosis, the temperature control sensor located on the transmission/hybrid drive unit has failed. Because Lexus designed this sensor as part of the transmission assembly, the dealer has stated the sensor cannot be replaced separately and that the only repair is to replace the entire transmission. Lexus is refusing to acknowledge this as a manufacturing defect and is requiring me, the consumer, to pay the full cost of a complete transmission replacement, estimated at approximately $9,040.03, solely due to the failure of this internal temperature sensor. This is a critical component used to monitor drive motor/transmission temperature. A malfunctioning sensor of this type can lead to improper system operation, possible overheating, loss of power, or unpredictable behavior of the hybrid drive system while the vehicle is in motion, which presents a safety risk to occupants and other road users. This failure occurred during normal use on public roads and not due to any accident, abuse, or modification. I am requesting that NHTSA investigate whether there is a design or manufacturing defect in the drive motor/transmission temperature sensor and its housing or circuitry on the 2020 Lexus RX 450h (and related Toyota/Lexus hybrid models) that forces owners to replace the entire transmission when this internal sensor fails. This appears to be an unreasonable safety‑related defect and an unreasonable repair burden on consumers.

Tires installed by Lexus on Sept 27, 2022 are shredding and coming apart in chunks at 37,000 miles; tire is rated 60,000 miles. This is 56% of the projected life of the tire. Previous tires, same brand and size, only lasted 29,000 miles; 1/2 of the life of the tire. Current tires have good tread depth and life left if the chunks were not uniformly coming off of all tires. This condition leaves the tire vulnerable to road hazards and moderately sharp objects. Failure is clearly imminent. Dealer, upon inspection July 1, 2025, did suggest immediate replacement of the tires at normal retail prices. No warnings or alerts or other symptoms are present. There has been a slight loss of air pressure.

I was driving down the road with no one around and nothing falling or being kicked up in the air and I heard a huge crash, I had no idea what happened even after stopping at a gas station. Two days later after hearing the sound of ripping Velcro as I was driving, I opened the sunroof bc it was a nice day and glass shattered and fell all over me. I shut to sunroof cover and realized the entire time that my sunroof exploded without warning bc it was not tempered correctly. Lexus refuses to fix the manufacturer error.

A safety recall was issued for my Lexus RX450h on 12/19/2023. I immediately contacted the dealership from whom I purchase the vehicle from. They informed me that "the part was on back order and would be there in September. On 09/12/2024, my partner took the vehicle in due to a nail in the tire and additionally to schedule for the vehicle to have the safety recall repaired, as they told us they would have the parts in September. I was already quite upset that they weren't going to have the parts for over 9 months after the safety recall was issued, especially one as important as an airbag, as this could be the difference between someone living or dying in a car accident. When we went back to the dealership, they told my partner that it was "more of a passive recommendation than a recall and that it was going to be updated next month about what is actually supposed to be done." I find this very concerning especially given that it is endangering the life of whomever sits in the passenger side of my vehicle. I don't know who to contact for help the dealership as it has become evident they have no intention of assisting me in this very important safety matter. I'm attaching a photo of the recall that still shows up on my app. I also have Tire & Wheel protection and my tire had a nail in it and it was taken to have a plug put in it and my partner was told that the leak (after pulling out the nail) was plugged. This was Thursday September 12th. However, I'm attaching a photo showing how much air had leaked out of the "plugged tired" Just a note - This is in addition to telling me that things needed to be done that didn't (which I found out from other auto body shop who reviewed the service record) and them lying to me about what services had been recently completed.
