THE CENTER CONTROL SCREEN WENT BLANK AND COULD NOT BE REBOOTED. IT WAS DETERMINED BY TESLA THAT THE MCU COMPUTER FAILED AND HAD TO BE REPLACED. MY FOUR YEAR WARRANTY HAD EXPIRED THE PREVIOUS MONTH SO THE COST TO REPLACE WAS $1,680.25. THOUGH THE CAR WAS DRIVABLE THE TURN INDICATORS WOULD NOT WORK. YOU SHOULD KNOW THAT THIS IS A VERY COMMON FAILURE ON ALL TESLAS THAT ARE FOUR YEARS OLD. THIS CAR ONLY HAD 47,770 MILES ON IT AT FAILURE. IT FAILED AT STARTUP. *TR

TL* THE CONTACT OWNS A 2016 TESLA MODEL S. THE CONTACT STATED THAT THE FAILURE OCCURRED WHEN THE ELECTRONIC OWNERS MANUAL COULD NOT BE ACCESSED ON THE INFOTAINMENT SCREEN. OVER TIME, THE SCREEN WOULD INTERMITTENTLY GO BLANK AND OTHER FEATURES ON THE HOME SCREEN WOULD NO LONGER FUNCTION AS NEEDED. THE CONTACT HAD TAKEN THE VEHICLE TO A TESLA FACTORY SERVICE CENTER WHERE HE WAS INFORMED THAT A REMEDY FOR THE DEFECT WAS NOT AVAILABLE HOWEVER, THE TECHNICIAN WOULD PROVIDE A SOFTWARE UPDATE. AFTER SEVERAL SOFTWARE UPDATES, THE FAILURE PERSISTED. THE MANUFACTURER WAS NOTIFIED OF THE FAILURE. THE VEHICLE WAS NOT REPAIRED. THE FAILURE MILEAGE WAS APPROXIMATELY 29,000.*DT CONSUMER HAS BROUGHT THE VEHICLE TO THE DEALER 7+TIMES, THERE WAS NO INDICATION OF UPGRADE OF THE SOFTWARE.*JB
MAIN CONTROL SCREEN ROUTINELY WENT BLACK. RENDERS THE AC AND OTHER CRITICAL VEHICLE CONTROLS USELESS. THIS IS A KNOWN FAILURE FOR MCU1. THIS HAPPENED MOSTLY WHILE DRIVING. SCREEN WAS REPLACED BY TESLA WITH REFURBISHED MODEL WHICH WILL FAIL THE SAME WAY. RATHER THAN REPLACING WITH MCU2 WHICH DOES NOT FAIL IN THIS MANNER.
MY CENTER SCREEN WENT BLACK (MCU FAILURE) WHILE DRIVING ON A LONG TRIP. I WAS UNABLE TO SEE THE BACKUP CAMERA OR PARKING SENSORS. MY AEB DID NOT PRODUCE AN AUDIBLE WARNING WHEN ACTIVATED. I COULD NOT USE HANDS FREE BLUETOOTH FOR CALLING. I COULD NOT ADJUST CLIMATE CONTROL.

The contact received a recall notification for NHTSA Campaign Number: 21V035000 (Back Over Prevention, Visibility, Exterior Lighting) however, the manufacturer had not returned his calls for the recall repair. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was notified of the issue. The vehicle was not diagnosed nor repaired. The approximate failure mileage was 38,000. Parts distribution disconnect.
MCU1 SHOWS SIGN OF FAILURE BUT NO OPTION OTHER THAN PAY OVER 1000 DOLLARS FOR REBEBRISH PART FROM TESLA OR UPGRADE TO CU2 TO ENSURE BEING IN DANGER IN THE FUTURE FOR $2500 0LUS TAX REGARDLESS YOU HAVE WARRANTY OR NOT.
IN LATE 2019, THE CENTER TOUCHSCREEN BEGAN SLOWING IN RESPONSIVENESS AND FREEZING AT TIMES. THE DASHBOARD SCREEN SOMETIMES WENT COMPLETELY BLACK, REQUIRING A REBOOT. WHEN THE CENTER TOUCHSREEN MALFUNCTIONED, THE BACKUP CAMERA WOULD NOT COME ON WHEN IN REVERSE GEAR. THE FREEZING AND SLOW RESPONSE TO BUTTON PRESSES CONTINUED TO DETERIORATE. THE WHOLE CONTROL SYSTEM (BOTH SCREENS) WOULD GO DARK AND REQUIRE A REBOOT SEVERAL TIMES A DAY. TESLA CLAIMED THAT THEY "COULD NOT REPRODUCE" THE PROBLEMS. I FINALLY PAID $2500 PLUS TAX FOR TESLA TO UPGRADE THE MAIN CONTROL UNIT FROM MY VERSION 1 TO THE NEWER VERSION 2. *TR

MY FRONT SUSPENSION IS CLUNKING AND CHATTERING AND CAUSING THE STEERING WHEEL TO VIBRATE AT LOW SPEEDS. TESLA HAS DENIED IT TO BE COVERED UNDER WARRANTY.

CENTER CONTROL PANEL IS NOT VERY RESPONSIVE AND FREEZES OFTEN. MANY OF THE MAIN CONTROLS FOR THE CAR ARE ON THE INSTRUMENT CENTER PANEL. I RECEIVED A RECALL NOTICE FROM TESLA BUT THEY REFUSE TO CORRECT THE PROBLEM AND KEEP SAYING PARTS ARE NOT AVAILABLE. IT IS HAS BEEN 3 MONTHS SINCE I RECEIVE THE NOTICE AND I FEEL THEY ARE DELAYING CORRECTING THE KNOWN ISSUE WITH EMMC DUAGHTERBOARD CHANGE.

THE MCU FAILED RENDERING THE VEHICLE UNSAFE TO DRIVE.
FAILURE OF TOUCHSCREEN OF 2016 TESLA MODEL S #128735 (MARCH 2016 BUILD). WAS TOLD BY TESLA MCU (MEDIA CONTROL UNIT) NEEDED TO BE REPLACED BECAUSE FLASH DRIVE WAS FILLED. I HAD THE MCU REPAIRED BY A 3RD PARTY REPAIR FACILITY SPECIALIZING IN TESLA'S WHERE THE FLASH DRIVE WAS REPLACED WITH A LARGER CHIP. I WOULD HOPE THAT OWNERS WHOM HAD TO HAVE THIS PART REPLACED EITHER BY TESLA OR 3RD PARTY WOULD BE COMPENSATED FOR THEIR OUT OF POCKET EXPENSES. I CHOSE 3RD PARTY REPAIR BECAUSE THEIR FIX IS PERMANENT, VS TESLA WHOM WAS GOING TO REPLACE MY MCU WITH THE SAME PART, GIVING IT ANOTHER FINITE PERIOD OF TIME BEFORE IT FAILS AGAIN. TESLA EVEN ADMITTED TO ME ON THE PHONE THAT THE MCU HAS A LIMITED LIFE CYCLE OF 3-5 YEARS. THE MAIN COMPUTER PANEL FAILED, GIVING NO ACCESS TO CONTROLS, CAMERA, HEAT AND AIR, AUTO BRAKING. AFTER THE 3RD TIME OF FAILURE THE MAIN COMPUTER PANEL STOPPED WORKING COMPLETELY. LEAVING IT UNSAFE TO DRIVE. THE VEHICLE IN THE FIRST 2 INSTANCES FAILED WHILE DRIVING AND A HARD RESET OF THE COMPUTER FIXED THE PROBLEM. THE 3RD TIME IT STOPPED WORKING UPON STARTUP WHERE I HAD A 100 MILE DRIVE HOME AND DID HAVE TO DRIVE WITHOUT THE USE OF THE MAIN COMPUTER SCREEN. I TOOK THE VEHICLE TO TESLA AND HAD TO HAVE THE ENTIRE MODULE REPLACED FOR OVER $2000.00. *TR
MAIN CONTROL UNIT FAILED MAKING THE ENTIRE CAR UNABLE TO DRIVE. THIS IS A KNOWN ISSUE BY TESLA. EVERY CAR WITH AN MCU1 WILL EVENTUALLY AND PREMATURELY FAIL.

A POLICE REPORT WAS FILED. THE AIR BAGS DEPLOYED. THE CONTACT STATED THAT THERE WAS A HEAD AND FOOT INJURIES SUSTAIN BY THE PEDESTRIANS THAT NEEDED MEDICAL ATTENTION. THE VEHICLE WAS TOWED TO THE CONTACT'S RESIDENCE. THE DEALER WAS NOT CONTACTED. THE VEHICLE WAS NOT REPAIRED. THE CONTACT ATTEMPTED TO CONTACT THE MANUFACTURER HOWEVER, THERE WAS NO ANSWER. THE APPROXIMATE FAILURE MILEAGE WAS 7,000.
The contact owns a 2016 Tesla Model S. The contact stated that while driving at an unknown speed, the electrical system intermittently failed. The contact stated that the turn signals failed to sound, the back-up camera was blank or was sometimes distorted. The contact stated that he was concerned for his safety while driving. No warning lights were illuminated. Additionally, the contact stated that two years ago, the vehicle performed a software update and afterwards, the electronics failed to properly operate. The vehicle was taken to the dealer where it was diagnosed that the failure was a known issue, and that the contact needed to wait for a software update. The vehicle was not repaired. The manufacturer was notified of the failure and the contact was informed to wait for the software update but no further assistance was provided. The failure mileage was unavailable.
FIRST TIME: MCU WENT BLANK 2 25 2020 2016.5 MS 75D 92,180K MILES MFD BY TESLA 7/16 BUILD DATE. DRIVIVG TO TESLA SERVICE CENTER CHARLOTTE, NC. FIRST REPAIR 25 FEB 2020: "CONCERN: CUSTOMER STATES THE TOUCHSCREEN IS BLANK CONFIRMED MCU WAS BLANK AND DID NOT RESPOND. PERFORMED 12V RESET AND DISPLAY DID NOT COME BACK ON. NECESSARY TO MANUALLY INSTALL FIRMWARE UPDATE AND THEN INSTALL A SECOND PACKAGE USING OVER THE AIR METHOD. FIRMWARE PACKAGES SUCCESSFULLY INSTALLED AND CENTER DISPLAY RETURN TO OPERATION. HIGH MEMORY USAGE WAS DUE TO SATELLITE MAP DATA. IF THIS SYMPTOM RETURNS, REPLACEMENT OF THE MCU WILL BE NECESSARY. CORRECTION: GENERAL DIAGNOSIS PRICE $175.00 SECOND TIME: MCU WENT BLANK 3 30 2020 TESLA SERVICE CENTER REMOVED AND REPLACED THE MCU. TOTAL COST TO ME AMOUNT ...$1,544.40. MY SAFETY ISSUE WAS THE AC AND HEAT AND DEFROST INOPERATIVE. ALONG WITH MY MODEL S UNABLE TO CHARGE AND LOOSING CHARGE. WHEN MY BLACK SCREEN HAPPENED MY AC/HEAT DID NOT GO TO DEFAULT...IT JUST DIDN'T WORK. I LIVE ON A MOUNTAIN AND WHEN I WAS DRIVING TO THE TESLA SC IN CHARLOTTE 117 MILES AWAY, ON A COLD MORNING THE WINDSCREEN FOGGED UP. I HAD TO WIPE IT WITH A RAG ON A STICK. 2016 TESLA MODEL S SUSPENSION BROKE OFF WHILE BACKING UP SLOWLY (5MPH) ON MY GARAGE. THE WHEEL END UP SCRAPING THE WHEEL WALL AND BECAME NOT DRIVABLE. TOOK THE CAR TO TESLA AND THEY MADE ME PAY $3150 FOR REPAIR. IT'S USUAL A SUSPENSION WOULD BREAK OFF AT SUPER SLOW SPEED ON REVERSE.

I WAS TRAVELING ON A TWO-LANE DIVIDED HIGHWAY WHEN I ACCELERATED TO PASS A VEHICLE TO MY RIGHT. AS I PASSED THE VEHICLE AND RELEASED MY FOOT OFF THE ACCELERATOR PEDAL, MY CAR CONTINUED TO ACCELERATE AT A HIGH RATE OF SPEED. UNSURE OF THE REASON FOR THIS CONTINUED ACCELERATION, I PUT THE TRANSMISSION INTO NEUTRAL AND COASTED TO MATCH THE SPEED OF THE SURROUNDING TRAFFIC. ARRIVING AT THE NEXT TRAFFIC LIGHT, I PUT THE TRANSMISSION BACK INTO DRIVE AND THE RAPID ACCELERATION STOPPED. I WAS ABLE TO GO BACK TO MY HOME AND INSPECT THE ACCELERATOR PEDAL AND ITS SURROUNDINGS. UPON INSPECTING THESE, I OBSERVED THAT THE DRIVER'S SIDE FLOOR MAT DIRECTLY BELOW THE ACCELERATOR PEDAL WAS RAISED UP FROM ITS NORMALLY FLAT POSITION ON THE CAR FLOOR ENOUGH TO CATCH THE ACCELERATOR PEDAL AND KEEP IT DEPRESSED. WHEN I TURNED OVER THE FLOOR MAT, I SAW THAT A CIRCULAR PIECE OF VELCRO ATTACHED TO THE UNDERSIDE OF THE FLOOR MAT, HAD LOST ITS ADHESIVENESS CAUSING IT TO DETACH FROM THE FLOOR MAT AND BUNCH UP UNDER THE FLOOR MAT BELOW THE ACCELERATOR PEDAL. THIS BUNCHING UP WAS ENOUGH TO RAISE THE FLOOR MAT TO A SUFFICIENT HEIGHT TO CATCH THE PEDAL. HAD I NOT KEPT COOL AND PUT THE TRANSMISSION INTO NEUTRAL, I SHUDDER TO THINK OF THE WRECKAGE AND CARNAGE WHICH MAY HAVE ENSUED. THIS CAR ADVERTISES ACCELERATION AT A VERY HIGH RATE OF SPEED; 0-60 MPH IN A MERE 5.5 SECONDS. THAT IS HARDLY ENOUGH TIME TO REACT TO SUCH A FRIGHTENING SITUATION! CURIOUS ABOUT HOW OTHER CAR BRANDS DEAL WITH DRIVER'S SIDE FLOOR MATS, I NOTICED THAT MOST CARS DRIVER'S SIDE FLOOR MATS ARE SAFELY CLEAR OF THE ACCELERATOR PEDALS. NOT SO WITH MY 2016 TESLA MODEL S. I BELIEVE IT TO BE A MAJOR DESIGN FLAW WHICH SHOULD BE REVIEWED EXPEDITIOUSLY AND CORRECTED BY TESLA AS SOON AS POSSIBLE BEFORE A TRAGIC OUTCOME ENSUES. THIS ISSUE HAS BEEN WELL DOCUMENTED AMONGST MANY OWNERS AND THE RESOLUTION IS TO ADJUST THE RUBBER DISCS THAT COME INTO CONTACT WITH THE PLASTIC ADJUSTABLE SHIMS OF THE CAR BODY. EVEN MORE DISTURBING, THE ALUMINUM BODY PANEL THAT SURROUNDS THE LIFT GATE OPENING IS EXHIBITING A SIGNIFICANT TENSION STRESS. I HAVEN'T REMOVED ANY INTERIOR BODY TRIM PANELS TO EXAMINE THE ADJACENT PANELS. I MEASURED THE BODY GAPS RESULTING FROM THIS STRUCTURAL FAILURE WITH A DIGITAL CALIPER. THE DIFFERENCE IN SPACING IS 3MM. THIS IS SIGNIFICANT. I'VE ATTACHED PHOTOS OF THE THREADED ROD AREA SHOWING SIGNS OF DUCTILE FRACTURE, THE RIGHT SIDE OF THE LIFT GATE OPENING EXHIBITING SIGNS OF FAILURE FROM THE TENSION STRESSES. I WAS APPROACHING A STOP SIGN WITH MY FOOT LIFTED ABOVE BOTH PEDALS (BRAKE AND ACCELERATOR) WHEN MY CAR ACCELERATED FORWARD. ON 1/18/20, I WAS TRAVELLING IN MY 2016 TESLA MODEL S P100D EASTWARD ON ANDERSON HILL ROAD TOWARD THE INTERSECTION WITH PURCHASE STREET. THE STOP LIGHT WAS RED. I WAS DRIVING 1MI UNDER THE SPEED LIMIT OF 35MPH. THE OUTSIDE TEMPERATURE WAS AROUND FREEZING, AND A WET SNOW HAD BEGUN 5-10MIN EARLIER, WITH MINIMAL ACCUMULATION. I KNOW EXACTLY HOW FAST I WAS GOING BECAUSE I HAD CRUISE CONTROL ON. I WAS NOT ON AUTOPILOT. JUST CRUISE CONTROL. WHEN THE INTERSECTION AND STOP LIGHT CAME INTO VIEW, I TAPPED THE BREAK TO DISENGAGE THE CRUISE CONTROL, AND THEN DEPRESSED THE BREAK TO SLOW THE CAR. NOTHING HAPPENED. THE BREAKS WOULD NOT ENGAGE. Sir/Madam, I own a Tesla Model S 2016 year car. I bought it in Nov 2018 from Tesla dealership. The car had free super charging and all warranty. In a parking lot entrance, someone rear ended my car and affected car back bumper and slight dent on quarter panel. The other car may not have traveled more than 5 miles as he was turning inside parking lot. The insurance company totaled my car saying Tesla won't provide parts quickly to repair. Since they can't wait, they totaled the car. Insurance company gave the car to us after deducting salvage value. Tesla didn't do body work in our area. So I took it to another shop and repaired the bumper and quarter panel. Due to pandemic we couldn't get rebuilt title quickly. Since my car was fully insured, insurance company said we can drive the car. While we were waiting for rebuilt title, I continue to use super charging and other work from Tesla. Once the salvage title entered into the DMV system, Tesla had access to it and since then, they removed all warranty and super charging. I am a realtor and use this car very much for my business. I explained to Tesla officers the need to have super charging, but they are saying once the car has rebuilt title, they will remove super charging capability. I told them I was using super charging it after my car was fixed, but they didn't care. I asked them if they can inspect the car for safety issue and they don't want to do it. Now my problem is I couldn't super charge even in other providers super charging location also. I provided all the details about the fix, rebuilt title etc. Even if they don't want to provide warranty, I certainly need super charging for free or by paying fee. I am wiling to do any safety check they want me to do. I am doing all other services in Tesla dealership only. I hope your department could intervene and help with situation.

FRONT WINDSHIELD HAS GHOSTING AFFECT WHEN VIEWING LIGHTS AT NIGHT. THIS IS MOST VISIBLE WITH LED LIGHTS SUCH AS NEW CAR TAIL LIGHTS AND NEWER TRAFFIC LIGHTS. LIGHTS APPEARS TO HAVE MULTIPLE VERSION OF THE SAME IMAGE WITH AN ADDITIONAL SET BEING ABOVE THE ACTUAL LIGHT AND ANOTHER SET BELOW. THESE ADDITIONAL LIGHTS MAKE IT VERY DISTRACTING AND DIFFICULT TO SEE WHEN THERE ARE MULTIPLE SOURCES OF LIGHTS SUCH AS HEAVY TRAFFIC. CAR WAS TESTED BY A TESLA SERVICE CENTER AND THEY SAID THE MULTIPLE IMAGES WERE WITHIN SPECS AND A REPLACEMENT WINDSHIELD WAS NOT NECESSARY. THE PROBLEM HAPPENS EVERY TIME THE CAR IS DRIVEN AT NIGHT WHEN OTHER CARS HAVE THEIR HEADLIGHTS AND TAIL LIGHTS TURNED ON. THE PROBLEM ONLY HAPPENS WHEN VIEWING THROUGH THE FRONT WINDSHIELD.
DOORS RANDOMLY POP OPEN. WHEN ENTERING THE CAR FROM ANY DOOR, THE FRONT PASSENGER SIDE DOOR AND THE REAR DRIVER'S SIDE DOOR POP OPEN. THE DOORS ALSO POP OPEN ANYTIME THE CAR IS PUT INTO PARK. SOMEIMES IT IS NECESSARY TO PUT THE CAR INTO PARK WHILE IN TRAFFIC AT A STOP LIGHT, OR WHEN STOPPED IN TRAFFIC JAMS. THIS CAN HAPPEN ON CITY STREETS, HIGHWAYS, FRANKLY ANYWHERE. WHEN THE CAR IS PUT INTO PARK THE DOORS POP OPEN. THIS IS CLEARLY A SAFETY ISSUE IN TRAFFIC. THE CAR IS LOW MILES AND ONLY 3.5 YEARS OLD. 3 OUT OF 4 DOORS HAVE HAD ISSUES. I SEE ON TESLA FORUMS THAT I AM ONE OF MANY PEOPLE THIS HAPPENS TO, SHORTLY AFTER THE WARRANTY EXPIRES, IN SOME CASES SOONER ALL OF THESE OWNERS FEEL THE SAME WAY I DO, THAT THERE ARE CLEARLY FAULTY DOORS THAT TESLA NOT ADDRESSING AS A SAFETY RECALL ISSUE. HAVING THE FRONT PASSENGER DOOR POP OPEN IN TRAFFIC IS ALARMING AND CONCERNING TO THE ELDERLY PEOPLE IN THE SEAT. ADDITIONALLY, THE REAR PASSENGER SIDE DOOR CAN NOT BE OPENED FROM THE OUTSIDE. THE MOTORS IN THE DOORS ARE ROUTINELY DISCUSSED IN TESLA FORUMS AS HAVING PROBLEMS. AGAIN, THIS IS A SAFETY ISSUE. IF THERE WAS AN EMERGENCY IN WHICH THE DOOR NEEDED TO BE OPENED FROM THE OUTSIDE TO EVACUATE MY CHILDREN FROM THE BACKSEAT, THIS WOULD NOT BE ABLE TO HAPPEN, AS THE HANDLE WILL NOT EVEN COME OUT TO OPEN THE DOOR. THESE ARE TOO MANY ISSUES ON A CAR THIS YOUNG AND PRESENT REAL AND KNOWN SAFETY ISSUES THAT ARE NOT BEING PROPERLY ADDRESSED BY TESLA.

Immediately after purchase of the vehicle from Tesla and on my way home from the dealer, I rolled my driver-side window down, then attempted to roll the window up upon entering the freeway. The window went half-way up, then rolled back down automatically. I attempted many times to roll the window up, but it would not go up. On the freeway, this caused a safety issue due to wind and debris entering inside the vehicle and causing safety problems for me. I took the car back to Tesla for repair. They indicated they fixed the problem, but this issue has occurred 6 more times, and each time I take the car in, they indicate it has been fixed, while the problem occurs again, not just with the driver-side window, but with other windows as well. I am seriously concerned about my safety and the safety of my family. This issue has occurred in various situations causing serious safety issues: 1. I took my car to Big Bear to ski, and the problem occurred in extremely cold temperature, exposing me and my family to extreme temperatures while attempting to operate the vehicle with the window down and unable to maintain cabin temperature for a few hours while trying to come back from the trip. 2. In two instances this issue occurred in the rain, causing water to enter the cabin and interfering with visibility and safely operating the vehicle. 3. In one instance this issue occurred while I was passing a place where landscaping sprinklers were on and I was sprayed in the face with water, seriously jeopardizing my visibility and I had to stop in place to clear my face from water before being able to see again and safely resume operation of the vehicle. 4. There are many more instances. Tesla is refusing to take back the vehicle as a lemon, despite repairing the vehicle 7 times all together, with the problem still occurring intermittently. I refuse to let my family drive the vehicle for fear of their safety, and I only drive it myself under very limited circumstances.
I'VE BEEN HAVING ONGOING ISSUES WITH THE MCU/ TOUCH SCREEN. I WAS DRIVING EARLY MORNING ONE DAY WHEN IT WAS STILL DARK OUTSIDE. I WENT TO TURN ON FOG LIGHTS AND ALL OF A SUDDEN THE HEADLIGHTS TURNED OFF AND THE TOUCH SCREEN FROZE ON ME. I HAD NO WAY OF TURNING THE HEADLIGHTS BACK ON AND HAD TO DRIVE IN THE DARK WITHOUT KNOWING WHAT IS AHEAD OF ME UNTIL I STOPPED ON THE SIDE OF THE ROAD. THEY DID REPLACE THE MCU, HOWEVER I'VE STILL BEEN HAVING ISSUES WITH THE TOUCH SCREEN. I DON'T HAVE FULL CONFIDENCE IN THE SYSTEM. HOW DO I KNOW THIS WON'T HAPPEN AGAIN?

IT HAS TO BE CONSIDERED AS A STRONG POSSIBILITY THAT IT WAS THE WHEEL GOING WHOMPY THAT CAUSED THE CAR TO SWERVE AND NARROWLY MISS IMPALING THE DRIVER.
