Southeast Toyota Distributors, LLC (SET) is recalling certain 2005-2011 4Runner, Highlander, Sequoia, and Sienna, 2005-2010 Avalon and Tundra, 2006-2008 Camry Solara, 2006-2010 Highlander hybrid, 2006-2011 Rav4 and Tacoma, 2007-2010 FJ Cruiser, 2007-2011 Camry hybrid, 2009-2010 Venza, 2010-2012 Prius, 1988-1990 and 2005-2010 Camry, and 1989 and 2009-2010 Corolla vehicles equipped with aftermarket accessory seat heaters with a copper strand heating element. The electrical wiring in the seat heaters may be damaged when the seat cushion is compressed.
Remedy Status
SET will notify owners, and dealers will disconnect the seat heaters, free of charge, and refund the purchase price of the seat heater accessory. The recall began on July 14, 2016. Owners may contact SET customer service at 1-866-405-4226. SET number for this recall is SET16B.
Toyota Motor Company (Toyota) is recalling certain model year 2009-2011 Tundra, Sequoia, Corolla, Corolla Matrix and Scion xB, 2008-2011 Highlander and Highlander Hybrid, 2007 Camry and Camry Hybrid, 2009 Camry and Camry Hybrid, 2006-2011 RAV4, 2006-2010 Yaris, and 2009-2010 Scion xD and Pontiac Vibe vehicles. During the manufacturing of the Power Window Master Switch (PWMS), grease lubricant may have been inconsistently applied to the sliding electrical contacts.
Remedy Status
Toyota will notify owners, and dealers will inspect the switch, applying a lubricant if no abnormality is found. If abnormality is found, the PWMS circuit board will be replaced, free of charge. The recall is expected to begin December 20, 2015. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is C0M.
Southeast Toyota Distributors, LLC (SET) is recalling certain model year 2010-2011 Prius and Corolla, 2009-2011 Venza, 2006-2010 Avalon, 2007-2010 FJ Cruiser, 2005-2011 Sienna and Sequoia, 2006-2011 Tacoma 4x2 and 4x4, Camry, Highlander, Tundra 4x2 and 4x4, and 2007-2011 Rav4 and 4Runner. The affected vehicles may experience compression of the seat cushion which may damage the seat heater wiring.
Remedy Status
SET will notify owners, and dealers will disconnect the heaters with copper strand heating elements and refund the purchase price of the seat heater accessory, free of charge. The recall is expected to began in January 2015. Owners may contact SET customer service at 1-866-405-4226.
Toyota is recalling certain model year 2007-2009 Camry, Camry Hybrid, RAV4, Corolla, Corolla Matrix, Tundra, Sequoia, Highlander, Highlander Hybrid, Yaris, Scion xB, Scion xD and Pontiac Vibe vehicles. The power window master switch assemblies in some of these vehicles were built using a less precise process for lubricating the internal components of the switch assemblies. Irregularities in this lubrication process may cause the power window master switch assemblies to malfunction and overheat.
Remedy Status
Toyota will notify its owners, and dealers will inspect the switch and apply a special grease that inhibits heat build up or replace any switch damaged by the defect, free of charge. Toyota owners that experience a problem with a switch should not attempt to make repairs, but should contact their Toyota dealer or the Toyota Customer Experience Center at 1-800-331-4331. General Motors will contact the Pontiac Vibe owners affected by this recall and GM dealers will inspect and repair switches as necessary. Those owners may contact Pontiac Customer Care at 1-800-762-2737.

IN THIS CONDITION, IF CONDENSATION OCCURS ON THE SURFACE, AS MAY OCCUR FROM HEATER OPERATION (WITHOUT A/C) WHEN THE PEDAL ASSEMBLY IS COLD, THE FRICTION WHEN THE ACCELERATOR PEDAL IS OPERATED MAY INCREASE, WHICH MAY RESULT IN THE ACCELERATOR PEDAL BECOMING HARDER TO DEPRESS, SLOWER TO RETURN, OR, IN THE WORST CASE, MECHANICALLY STUCK IN A PARTIALLY DEPRESSED POSITION.
Remedy Status
DEALERS WILL INSTALL A REINFORCEMENT BAR IN THE ACCELERATOR PEDAL WHICH WILL ALLOW THE PEDAL TO OPERATE SMOOTHLY. GM WILL NOTIFY OWNERS FOR THE PONTIAC VIBE PLEASE SEE 10V-018. THIS SERVICE WILL BE PERFORMED FREE OF CHARGE. THE SAFETY RECALL IS EXPECTED TO BEGIN ON FEBRUARY 5, 2010 AND WILL BE COMPLETED ON MARCH 15, 2010. OWNERS MAY CONTACT TOYOTA AT 1-800-331-4331.
TOYOTA IS RECALLING CERTAIN MODEL YEAR 2004-2010 PASSENGER VEHICLES. THE ACCELERATOR PEDAL CAN GET STUCK IN THE WIDE OPEN POSITION DUE TO ITS BEING TRAPPED BY AN UNSECURED OR INCOMPATIBLE DRIVER'S FLOOR MAT.
Remedy Status
TOYOTA FILED AN AMENDED DEFECT REPORT ON NOVEMBER 25, 2009, STATING THAT DEALERS WILL MODIFY THE ACCELERATOR PEDAL AND, ON CERTAIN VEHICLES, ALTER THE SHAPE OF THE FLOOR SURFACE UNDER THE PEDAL. THESE CHANGES ADDRESS THE RISK OF PEDAL ENTRAPMENT DUE TO INTERFERENCE WITH THE FLOOR MAT. REDESIGNED ACCELERATOR PEDALS WILL BECOME AVAILABLE BEGINNING IN APRIL 2010 AND DEALERS WILL REPLACE ANY MODIFIED PEDAL WITH THE NEW PEDAL IF DESIRED. ALSO, DEALERS WILL REPLACE ANY GENUINE TOYOTA OR LEXUS ALL-WEATHER FLOOR MATS WITH REDESIGNED ALL-WEATHER MATS, OR REPURCHASE THE PREVIOUS MATS FROM OWNERS WHO DO NOT WANT THE NEW ONES. ADDITIONALLY, SOFTWARE MODIFICATIONS WILL BE INSTALLED ON CAMRY, AVALON AND LEXUS ES 350, IS 350 AND IS 250 MODELS THAT WILL ENSURE THAT THE BRAKE OVERRIDES THE ACCELERATOR IN THE EVENT BOTH BRAKE AND ACCELERATOR PEDALS ARE APPLIED. TOYOTA WILL BEGIN MAILING LETTERS TO OWNERS ON DECEMBER 31, 2009. OWNERS MAY CONTACT TOYOTA AT 1-800-331-4331, LEXUS AT 1-800-255-3987.
CERTAIN ALL WEATHER FLOOR MATS (AWFM) SPECIFICALLY SOLD AS AN OPTIONAL ACCESSORY FOR USE ON THE 2007 AND 2008 MODEL YEAR LEXUS ES350 AND TOYOTA CAMRY VEHICLES. THE AWFM INCLUDES TWO GROMMET HOLES AND RETAINING HOOKS (CLIPS) TO SECURE THE MAT TO THE VEHICLE'S CARPET. IF THE AWFM IS NOT SECURED BY ITSELF OR IF IT IS PLACED ON TOP OF AN EXISTING CARPETING FLOOR MAT, THE MAT COULD MOVE FORWARD DURING THE VEHICLE USAGE AND IT MAY INTERFERE WITH THE ACCELERATOR PEDAL.
Remedy Status
TOYOTA WILL NOTIFY OWNERS OF ALL 2007 AND 2008 LEXUS ES350 AND TOYOTA CAMRY VEHICLES ABOUT THE RECALL EVEN THOUGH NOT ALL OF THESE OWNERS WILL HAVE PURCHASED THE ALL WEATHER MATS. IF THE OWNER HAD NOT PURCHASED THE MATS, THEY ARE NOT AFFECTED BY THE RECALL. THIS LETTER WILL ADVISE OWNERS WHEN REPLACEMENT MATS WILL BE AVAILABLE OR HOW TO RETURN THE ALL WEATHER FLOOR MAT SET TO OBTAIN A REFUND. A SECOND LETTER WILL BE SENT AFTER MATS ARE AVAILABLE REQUESTING OWNERS TO RETURN THEIR MATS TO A LEXUS OR TOYOTA DEALER FOR EXCHANGE OF THE DRIVER'S SIDE MAT. THE FIRST LETTER IS EXPECTED TO BE SENT DURING EARLY OCTOBER 2007 AND THE SECOND IN LATE NOVEMBER 2007. ON CERTAIN VEHICLES, DUE TO IMPROPER ASSEMBLY OF THE AIR BAG INFLATOR, WHICH IS USED IN THE SIDE AIR BAG, THE CURTAIN SHIELD AIR BAG, AND THE KNEE AIR BAG ASSEMBLY, SOME INFLATORS WERE PRODUCED WITH AN INSUFFICIENT AMOUNT OF THE HEATING AGENTS NECESSARY FOR PROPER AIR BAG DEPLOYMENT. IN THIS CONDITION, THE EXPANSION FORCE OF THE GAS MAY BE INSUFFICIENT TO PROPERLY INFLATE THE AIR BAG WHEN THE SRS SYSTEM IS ACTIVATED DURING A CRASH.
Remedy Status
DEALERS WILL REPLACE THE SPECIFIC SRS AIR BAG. THE RECALL BEGAN ON APRIL 6, 2006. OWNERS MAY CONTACT TOYOTA AT 1-888-270-9371, SCION AT 1-866-548-1851, OR LEXUS AT 1-800-255-3987.

THE DRIVER SIDE FLOOR MAT WILL NOT INTERFERE WITH THE THROTTLE PEDAL IF PROPERLY SECURED USING THE RETAINING HOOKS PROVIDED BY TOYOTA.HOWEVER, IF THE ALL WEATHER MAT IS UNSECURED AND MOVES FORWARD FROM ITS INTENDED POSITION IT CAN ENTRAP THE THROTTLE PEDAL AT THE FULLY OPEN POSITION AFTER IT HAS BEEN DEPRESSED.THIS CAN HAPPEN REGARDLESS OF WHETHER OR NOT ANOTHER MAT (CARPET) IS UNDERNEATH.WHEN THIS OCCURS, SOME OPERATORS REACT BY APPLYING THE BRAKE PEDAL MULTIPLE TIMES, DEPLETING THE BRAKING SYSTEM'S (VACUUM BASED) POWER ASSIST.STOPPING THE VEHICLE WITH UNASSISTED BRAKING WHILE THE THROTTLE IS FULLY OPEN REQUIRES SIGNIFICANT PEDAL FORCE, WHICH SOME OPERATORS DID NOT, OR WERE UNABLE TO, APPLY FOR THE REQUIRED DURATION.CONTINUED DRIVING IN THIS CONDITION RESULTS IN OVERHEATED BRAKES, WHICH FURTHER DIMINISHES THE BRAKING EFFECTIVENESS.SOME OPERATORS ATTEMPTED TO TURN THE VEHICLE OFF BY DEPRESSING THE ENGINE CONTROL BUTTON, HOWEVER THEY WERE UNAWARE THE BUTTON HAD TO BE DEPRESSED FOR THREE SECONDS TO STOP THE ENGINE WHEN THE VEHICLE IS IN MOTION; THIS FUNCTIONALITY WAS NOT EXPLAINED ADEQUATELY IN THE OWNER'S MANUAL.IN ODI INTERVIEWS, OWNER'S REPORTED THEY WERE UNAWARE THE ALL WEATHER MAT HAD THE POTENTIAL TO INTERFERE WITH THE THROTTLE PEDAL, ADVISING THAT THIS WAS NEVER PROPERLY EXPLAINED GIVEN THE SIGNIFICANT HAZARD IT REPRESENTED.OWNERS REPORTED SEVERAL EXPLANATIONS FOR WHY THE MAT WAS UNSECURED, INCLUDING THAT THE VEHICLE WAS DELIVERED IN THIS CONDITION (MOST COMMON REASON), OR THAT THEY HAD (UNWITTINGLY) INSTALLED THE MAT THEMSELVES, OR THAT ANOTHER PARTY, SUCH AS A CAR WASH ATTENDANT, HAD DISTURBED THE MAT WITHOUT THE OWNER'S KNOWLEDGE.ONE CONSUMER REPORTED THAT THEIR VEHICLE WAS DELIVERED WITHOUT THE RETAINING DEVICES INSTALLED.IN A SEPTEMBER 26, 2007 LETTER TO NHTSA, TOYOTA INDICATED THAT THEY WOULD CONDUCT A SAFETY RECALL TO REPLACE THE ALL WEATHER MAT WITH A REDESIGNED MAT.ACCORDING TO TOYOTA, THE NEW MAT DESIGN WILL REDUCE THE POTENTIAL FOR MAT INTERFERENCE WITH THE THROTTLE PEDAL.THE POPULATION NOTED ABOVE REPRESENTS THE NUMBER OF SUBJECT MATSTHAT HAVE BEEN SOLD IN THE UNITED STATES.SINCE TOYOTA CANNOT IDENTIFY WHICH VEHICLES MAY HAVE THE SUBJECT MATS, THEY WILL SEND A LETTER TO ALL REGISTERED OWNERS OF THE SUBJECT VEHICLES (ESTIMATED 750K) ADVISING THEM OF THE CONCERN AND REMEDY.A COPY OF THAT LETTER WILL BE POSTED TO NHTSA'S WEB SITE UNDER RECALL 07E-082; IT CONTAINS A DESCRIPTION OF THE CONDITION AND THE ACTIONS AN OPERATOR SHOULD TAKE IN THE EVENT THROTTLE ENTRAPMENT OCCURS WHILE DRIVING.THE FATALITY INCIDENT NOTED ABOVE OCCURRED IN JULY 2007 AND WAS INVESTIGATED BY NHTSA'S SPECIAL CRASH INVESTIGATIONS OFFICE.IT HAS NOT BEEN REPORTED ON A VEHICLE OWNER QUESTIONNAIRE.THE OPERATOR REPORTEDLY TRAVELLED AT SPEEDS IN EXCESS OF 100 MPH FOR AN ESTIMATED EIGHT MILES ON AN INTERSTATE IN CALIFORNIA BEFORE IT STRUCK TWO OTHER VEHICLES.ONE OF THE STRUCK VEHICLES AND THE SUBJECT VEHICLE CAUGHT FIRE.THE OCCUPANT OF THE STRUCK VEHICLE DID NOT EVACULATE AND DIED AT THE SCENE.THE SUBJECT VEHICLE DRIVER SUFFERED A BROKEN BONE.THE CALIFORNIA HIGHWAY PATROL IS INVESTIGATING THE INCIDENT ALSO.ODI DID NOT ISSUE AN INFORMATION REQUEST LETTER DURING THIS INVESTIGATION.THE TOYOTA REPORTS NOTED ABOVE WERE REPORTED DURING PRELIMINARY EVALUATION PE07-016 AND ARE CURRENT THROUGH APRIL 2007 FOR MY 2007 LEXUS ES350 ONLY.ODI DOES NOT HAVE FIELD EXPERIENCE DATA FROM TOYOTA FOR CAMRY VEHICLES.THROTTLE ENTRAPMENT DUE TO IMPROPERLY INSTALLED FLOOR MATS COULD BE A CONCERN IN ALL VEHICLES.THEREFORE, DRIVERS SHOULD ALWAYS ENSURE THEIR FLOOR MATS ARE PROPERLY AND SAFELY INSTALLED.THIS INCL
